Tara Carpenter is home alone when she receives a phone call from a mysterious voice claiming to be Charlie. The caller asks her horror movie questions and reveals he is outside her house, creating an immediate sense of danger. Tara attempts to text her friend Amber while being taunted and threatened by Ghostface, the killer.

Ghostface breaks into Tara’s house after she answers her final horror question wrong. He slashes her stomach, forcing her to call the police while he taunts her over the phone. Ghostface ultimately breaches the house, stabbing Tara multiple times and breaking her foot, but the police arrive just in time to scare him off.

Tara is transported to the hospital where her sister Sam and friends visit her in her room. Tara is happy to see Sam again after five years, but she asks everyone but her sister to leave, signaling a moment of emotional tension and concern for her recovery.

Sam and her boyfriend Richie learn about Tara’s attack and head back to Woodsboro, discussing the connection to the 'Stab' movies. They consider the possibility of the attack being related to the original murders and franchise history, setting the stage for their investigation.

After waking up, Tara receives a message from Wes Hicks, prompting her friends to visit her at the hospital. The group of friends gathers near Woodsboro High, discussing Tara’s condition and recent events, including the mysterious Creepy Vince trying to reconnect with Liv.

Vince, who had been trying to rekindle his relationship with Liv, finds his car tampered with and is later stalked and killed by Ghostface. He is stabbed in the neck and left bleeding to death, marking the killer’s first confirmed murder in the film’s sequence.

While at the hospital, Sam goes to the bathroom and experiences a hallucination of Billy Loomis, the original Woodsboro killer. He provides her with cryptic advice about the murders, warning her that she has a connection to the killer, heightening her suspicion and distress.

Sam confesses to Tara that she is Billy Loomis’s daughter and fled Woodsboro after discovering her true heritage. She explains her guilt and fear of becoming like Billy, which explains her distant behavior and adds complexity to her character.

Dewey Riley, a former sheriff, is convinced by Sam that the murders are linked to the original 'Stab' franchise. Despite initial reluctance, Dewey agrees to help and returns to Woodsboro with Sam and Richie, seeking to stop the killer and uncover the truth.

At Chad and Mindy’s home, the friends theorize that the killer is creating a 're-quel' of the 'Stab' movies, blending a reboot and sequel to incite fans. They link the murders to the franchise's legacy characters and suspect that the killer’s motives may involve revenge or a personal vendetta.

Wes Hicks, the son of Judy Hicks, is called away and murdered by Ghostface after Judy is attacked and killed at home. Wes’s murder confirms that the killer is targeting those connected to the original 'Stab' stories and their relatives.

Dewey confronts Ghostface at the hospital scene and shoots him three times, but Ghostface counterattacks, stabbing Dewey in the gut and back. Tragically, Dewey succumbs to his injuries, sacrificing himself to save others and cementing his legacy as a hero.

Sidney Prescott arrives in Woodsboro after hearing about Dewey’s death. She joins Gale and the others in confronting Ghostface inside Stu’s house. It is revealed that Amber and Richie are the killers, motivated by their disdain for the franchise and a desire for revenge.

In a final showdown, Amber and Richie reveal themselves as the killers. Amber is consumed by flames after a struggle, and Richie is stabbed and killed by Sam. Tara shoots and kills Amber, while Sam strikes and kills Richie to prevent him from reviving the killings.

The police and medics arrive, and the survivors—Chad and Mindy—are alive. Sam thanks Sidney and Gale for their help, and Gale decides her next book will honor Dewey’s heroism. Tara is taken to the hospital, and the film concludes with news coverage of the events, closing the chapter on the Woodsboro murders.
