The film begins with Seth trapped in a deadly pendulum mechanism that threatens to bisect him. He learns that to save himself, he must destroy the very tools of his murder—his own hands.

Despite realizing that escape may be impossible, Seth chooses to crush his hands in an attempt to save himself. Unfortunately, the pendulum continues its descent, leading to his gruesome demise.

Meanwhile, Agent Strahm is in pursuit of the truth following Jigsaw's death. He finds Jigsaw's lifeless body and, shortly thereafter, becomes trapped in a hidden room by fellow agent Mark Hoffman.

Strahm discovers a concealed door and hears a recorded warning from Jigsaw, cautioning him against his relentless search for truth. Ignoring the warning, he ventures into the mysterious corridor.

As Strahm explores further, he is ambushed by an assailant in a pig mask, believed to be Hoffman. Strahm finds himself in a trap with his head enclosed in a container filling with water.

In a life or death situation, Strahm performs a tracheotomy with a ballpoint pen to save himself from drowning. His desperate actions highlight the grim stakes he faces.

Law enforcement arrives at the Gideon Warehouse, where Hoffman is seen carrying Corbett, claiming he saved her. Strahm, though critically injured, is taken away on a stretcher.

The following day, a memorial service honors the victims of the Jigsaw murders. During this service, the Chief of Police announces the case's conclusion and promotes Hoffman.

Jill, Jigsaw's ex-wife, receives a mysterious videotape and box from John's attorney. The contents remain undisclosed, leaving a sense of foreboding as she leaves without revealing any details.

Strahm mourns the loss of his partner, Agent Perez, and is reassigned off the Jigsaw case by his superior, Agent Erickson. He feels powerless and frustrated as Hoffman's calm demeanor raises suspicion.

Meanwhile, five individuals wake up in a sewer, caught in a treacherous necktie trap. They must cooperate to retrieve keys to escape the looming danger of beheading as explosives tick down.

Tensions rise among the five as they frantically attempt to escape. Panic leads to rash decisions, resulting in one individual, Ashley, falling victim to the deadly guillotine.

Strahm investigates further, uncovering connections between Hoffman's sister’s murder and Seth’s fate. This revelation marks a dark turning point, demonstrating Hoffman's manipulation.

The five in the sewer face increasingly lethal challenges, including smashing jars for keys as bombs threaten to detonate. The climax reveals their failure to cooperate leads to tragic consequences.

In a chilling twist, Strahm engages in a final chase with Hoffman, leading him back to a house from the past. However, Strahm ultimately becomes ensnared in a trap, sealing his fate.
