Kate and Rick share a blissful and loving wedding, where they both express how safe they make one another feel. However, their perfect life is disrupted when Rick is deployed to Afghanistan as a military sergeant. During an intense battle, Rick finds himself isolated in a cave, where the walls are lined with mysterious arcane symbols that glow eerily. Suddenly, a shadowy figure appears behind him, startling Rick into a panic. Acting on instinct, he turns and fires his weapon, horrifyingly discovering that he has shot a Pashtun child, who now lies bleeding before him. In a surreal turn of events, the child's body erupts into a cloud of sand, enveloping Rick.
Back home, Kate and her father express their hopes for Rick's safe return. When Rick does return unexpectedly, seated behind Kate on a bench, he displays an unsettling emotional detachment, leaving Kate feeling confused and heartbroken. As nighttime falls, Kate is plagued by disturbing nightmares, including visions of stepping barefoot onto shattered glass next to her bed and of Rick inexplicably vanishing.
Concerned about Rick's well-being, a military psychiatrist diagnoses him with PTSD and recommends he join a therapy group. In this group, Rick and fellow veterans create unsettling masks, which they wear as a form of therapy. During an intimate moment at home, Kate becomes frightened when Rick, seemingly lost in his trauma, dons the mask while they are in bed, prompting her to ask him to stop.
Eventually, hope arrives in the form of a kind Imam from a local mosque, who offers to help Rick through a spiritual exorcism aimed at freeing him from the emotional burdens he carries. This encounter marks a pivotal moment in Rick's journey as he seeks to reclaim his identity and reconnect with Kate, who longs for the return of the man she married.

Refuge Plot Explained: Your Questions Answered
The questions people ask most about Refuge (2023): what happens in the confusing scenes, why the characters do what they do, the twists, hidden details and what the ending means. Each answer contains spoilers.
Q1 Why did Rick shoot the child in the cave in Refuge?
Rick shot the child out of pure combat reflex. After being separated from his unit and trapped in a dark cave lined with glowing, otherworldly symbols, a shadowy figure suddenly appeared behind him. His training as a military sergeant kicked in, and he fired before he could process what he was looking at, only to discover he had killed a Pashtun child.
Foreshadowing: The glowing arcane symbols on the cave walls establish the location as a place of supernatural significance, foreshadowing that Rick's combat instincts alone will not be enough to protect him from what lurks there.
- rick shoots child
- cave scene refuge
- pashtun child
- combat reflex
- military sergeant
Q2 What happened to the child after Rick shot him?
The child does not simply die. After being shot, the child's body erupts into a cloud of sand that envelops Rick, suggesting a supernatural or metaphysical dimension to the encounter. This transformation is the pivotal moment that fractures Rick's reality and sets the rest of the story's horror in motion.
- child turns to sand
- supernatural transformation
- sand cloud
- metaphysical encounter
Q3 What do the glowing symbols in the cave mean?
The symbols appear to be arcane or ritualistic markings, hinting that the cave is not an ordinary enemy hideout but a site with spiritual or supernatural significance. Their eerie glow signals that something otherworldly is present, which connects directly to the impossible transformation of the child's body moments later and explains why Rick's trauma takes on a demonic quality rather than a purely clinical one.
- cave symbols
- arcane markings
- supernatural cave
- glowing symbols refuge
Q4 Why is Rick so emotionally detached when he comes home?
Rick returns home unable to connect because the cave encounter broke something fundamental in him. Beyond the horror of killing a child, the supernatural transformation and the sand that enveloped him appear to have left a spiritual burden or possession attached to him, leaving him shut down, unreachable, and unable to be the husband Kate remembers.
- emotional detachment
- rick changed
- pTSD veteran behavior
- emotional numbness
Q5 Why does Kate start having nightmares after Rick returns?
Kate's nightmares mirror her subconscious fear that the husband who came back is not truly the man she married. Dreams of stepping on broken glass and watching Rick vanish again represent her anxiety about living with someone who is psychologically and spiritually altered, as well as her dread of losing him a second time.
- kate nightmares
- dreams of broken glass
- anxiety dreams
- fears of losing rick
Q6 Why do the veterans in the therapy group make and wear masks?
The mask-making is a non-traditional form of PTSD therapy designed to externalize the trauma the veterans carry inside. By creating physical faces to wear, the group attempts to confront and process the hidden identities combat has forced onto them, though the practice takes a darker turn when Rick continues wearing the mask outside the group setting.
- mask therapy
- veteran therapy group
- pTSD treatment
- trauma masks
Q7 Why does Rick wear a mask in bed with Kate?
Wearing the mask in an intimate moment shows that Rick's trauma has fully invaded his personal life and that he can no longer separate his therapy exercises from his identity as a husband. The mask has become a shield he cannot remove, which is what frightens Kate into asking him to stop and signals that clinical treatment is no longer enough to reach him.
- rick mask bedroom
- intimate scene mask
- mask in bed
- trauma invasion
Q8 Why does Rick need a spiritual exorcism instead of just therapy?
Although a military psychiatrist diagnoses Rick with PTSD, his symptoms behave less like a psychological condition and more like a spiritual affliction rooted in the cave encounter. Because something appears to have attached itself to him when the child transformed into sand, the Imam's exorcism is presented as the only path capable of addressing the supernatural layer that conventional therapy cannot touch.
- imam exorcism
- spiritual healing
- demonic possession
- exorcism scene
Q9 How is Rick's PTSD connected to something supernatural?
Rick's trauma cannot be explained by combat stress alone. The glowing symbols, the impossible transformation of the child into sand, and the way that sand enveloped him all suggest a malevolent spiritual force took hold of him in the cave. His PTSD symptoms, including the mask fixation and emotional disconnection, are framed as the visible expression of a deeper demonic attachment rather than purely psychological damage.
Foreshadowing: The Pashtun child's body turning into a drifting cloud of sand is the first clear signal that what is happening to Rick is not standard combat trauma but a metaphysical intrusion.
- supernatural ptsd
- demonic possession
- spiritual trauma
- metaphysical horror
Q10 Why does the Imam agree to help Rick?
The Imam steps in because Rick's suffering presents itself as a spiritual affliction rather than a purely medical one, and the Imam has the authority and tools to perform the kind of intervention required. His involvement marks the moment the story shifts from treating Rick's condition as clinical PTSD to treating it as a battle for his soul, which is what Kate and the military framework could not achieve.
- imam helps rick
- mosque intervention
- spiritual guidance
- community support
Q11 What does the exorcism mean for Rick and Kate's marriage?
The exorcism represents the couple's last hope of restoring the man and the marriage they once had. Because Rick's identity has been overtaken by the trauma and the spiritual burden he carries, freeing him from that force is framed as the only way Kate can get her husband back, and the only way Rick can reclaim himself.
- exorcism marriage
- rick and kate
- reclaim identity
- marriage redemption
Q12 Why does Kate ask Rick to stop wearing the mask?
Kate asks him to stop because the mask reveals that the Rick who came home is no longer the Rick she married. It signals that his trauma has overtaken his identity to the point where he cannot be emotionally present with her, and removing the mask becomes her way of demanding that the real Rick return to their relationship.
- kate asks rick to stop
- removing mask
- identity crisis
- marriage tension
