A Caribbean coastal resort, Hotel Elysium, finds itself menaced not by storms but by a deadly creature feature: flying piranhas spawned from a sunken shipwreck that lurks just off the coast.
Anne Kimbrough [Tricia O’Neil] is a diving instructor at the hotel who notices something off when a student becomes the latest victim. Her estranged police officer husband, Steve Kimbrough [Lance Henriksen], refuses to let her see the corpse, even as she knows the death wasn’t caused by a shark or barracuda—her marine biology training tells a different story, one that unsettles the couple’s fragile past. Soon after, a string of killings—two women and a man—confirms her fears: the attackers are piranhas that have somehow evolved the ability to fly.
Concerned and stubborn, Anne fixes her sights on a tourist, Tyler Sherman [Steve Marachuk], who constantly bugged her and who she tries to bring along to the morgue to inspect the body. When Anne tries to cancel diving sessions, her manager fires her in a blow to her livelihood. In a bid to prove the threat, she aims to capture a specimen, only to be intercepted by Tyler, who reveals he is a biochemist and part of a team that has created a weaponized, genetically modified piranha capable of flight. He explains that a cylinder of these fish was lost earlier, still somewhere in the water.
Gabby [Ancile Gloudon] provides crucial proof by showing that the piranhas are now eating each other, underscoring the peril. Anne and Tyler race to the wreck in a motorboat, planting Gabby’s timer charges. With only ten minutes to escape, they find themselves trapped in one of the ship’s rooms as the piranhas close in. Steve, piloting a police helicopter, ditches the chopper and swims to the motorboat, then powers it away toward safety with Chris and Allison aboard. Tyler becomes trapped in the wreck and is consumed by the hungry fish. Anne escapes through a porthole and grabs the ship’s anchor to haul herself free as the motorboat pulls away.
