Parasite Eve

Kiyomi visits Nagashima at his workplace for their anniversary, and he forgets the date. She leaves disappointed. On the way home, she is involved in a car accident and falls into a coma with brain damage.

The next day, Dr. Yoshizumi informs Nagashima that Kiyomi is an organ donor and that Mariko needs a donor kidney. Nagashima agrees to donate Mariko's kidneys only if he can obtain Kiyomi's liver for his experiments.

Kiyomi's liver is taken to Nagashima's private lab, and he dismisses the other scientists to work alone. He begins planning experiments to test the organ's regenerative potential.

At the hospital, Mariko's condition shows marked improvement, but at night she screams and demands the removal of something from her body. The caretakers dismiss the voices as nightmares.

In the lab, the liver samples grow rapidly. A gelatinous substance from the liver mixes with a bottle labeled Eve and possesses the lab assistant Sachiko.

The gelatin transforms into a living version of Kiyomi, and Nagashima embraces and has sex with the apparition on the lab floor. The being then deteriorates, leaving him alone.

Back home, Nagashima finds Kiyomi's journals describing how she was losing control and hearing voices that steered her toward his experiments. The entries imply she manipulated events to push him forward.

Sachiko invites Nagashima to her science conference presentation. Yoshizumi reports that Mariko has grown stranger, with changes including her uterus transforming.

During the conference, Sachiko unveils herself as part of a mitochondria collective aiming to replace humans and control their mitochondria. The group claims an ideal womb for continued evolution. Nagashima questions whether the figure is Eve.

The creature causes a man in the audience to burst into flames through mitochondria, triggering panic and a rapid evacuation. The surreal attack confirms the threat the collective poses.

At the hospital, Mariko's abdomen convulses as staff scramble to help. The creature uses the moment to attempt escape with Mariko, slipping away through the hospital corridors toward the roof.

The creature moves to the roof with Mariko. Nagashima pleads, calling the creature Kiyomi to release the girl. It declares a new evolution and that Mariko will bear the real Mitochondrial Eve, while Nagashima's arms catch fire in the confrontation.

Nagashima and the creature burn as they embrace Kiyomi. Takashi arrives on the roof and rescues Mariko, but the rooftop is engulfed in flames, sealing their fates.
