Ganesh [Narain] is a slum-born auto driver who sends money home to support his family. Bhuvana [Poonam Kaur] is the neglected daughter of a rich man, Rangasamy, and she is in search of true love. Drawn to Ganesh’s honesty and good heart, she confesses her feelings, but he turns her away, saying that a rich girl like her could never live a life of poverty with him. Bhuvana insists she can prove otherwise and arrives at Ganesh’s house with only the clothes on her back. What starts as a heartfelt romance soon meets a devastating test that threatens to upend their fragile happiness.

As Ganesh and Bhuvana share a happy life and mutual understanding, life is all roses until a storm disrupts their happiness. Bhuvana suffers an accident and fights for her life. The doctors tell them that nothing but a heart transplant could save her. Ganesh has no money to spare. He scrambles to raise funds, collecting about two lakhs out of the required three, but the hospital refuses to accept it. In a desperate move, he storms KRS, the hospital MD, at a VIP party and creates a scene. He is arrested and detained in a room, but his frustration erupts into a hostage situation in the emergency room, where he threatens to blow up the 40 people inside.

The hostages—among them the cardiologist who refused to operate, a minister hiding from journalists, a child with leukemia, an old couple, and a young couple—are soon drawn into Ganesh’s plight. An accident emergency case arrives, and Ganesh opens the room for treatment. The hostages grow sympathetic to his cause and begin to doubt his threats. The police fail to resolve the crisis, yet they end up yielding to his demand and decide to operate on Bhuvana, showing a shift in the room’s mood.

Ayyanar, the DCP, seeks revenge for being slapped by Ganesh during one of the hostage talks and pretends to be an injured victim to gain entry. The hostages resist leaving the room even as Ganesh is apprehended. Although the hospital agrees to operate, no hospital seems to have a matching heart for the transplant. This is when Ganesh decides to give his own heart. He cuts his hand to send for a blood group, and it matches Bhuvana’s blood type. The doctor refuses to accept a living heart, calling it illegal. Facing a grim choice, Ganesh resolves to sacrifice himself for her, begging the others to find another way. After a final conversation with Bhuvana, he chooses to end his life and donates his heart to her, sealing his love through the ultimate act of selflessness.
