Max Matheson survives a car crash that kills his fiancée Kayla, leaving him with survivor's guilt and ongoing trauma. He undergoes psychological treatment with Dr. Beaumont and slowly makes progress, though memories of the crash haunt him.

Max's father, Jack, reopens the Mayflower Department Store in New Orleans and hires Max as a security guard, replacing the former guard who quit after injuring himself on duty. Max is introduced to the store's leadership—Keller Landreaux, Jenna McCarty, and Ryan Parker—and accepts the job as a step toward rebuilding his life. He stays wary of the new environment as he begins his shift.

Before his first shift, Max experiences a vision of a dead woman in a mirror, and Jenna's naked reflection is seen ripping off her head. That night, Jenna dies when a shard of glass decapitates her in the shower, tying her fate to Max's visions. Max realizes he foresees deaths through mirrors.

The next night, Max sees Ryan's reflection being eviscerated in a mirror and attempts to warn him. He arrives too late, and Ryan dies in the same way, confirming the visions predict real fatalities. Max rushes to Ryan's apartment but can't save him.

Max calls his father to quit the job, but he soon finds Jack's face covered in cuts and races to his house. Jack's reflection almost kills him, and Max intervenes to save his father. This near-death encounter convinces Max to stay at the Mayflower and confront the haunting.

Back at the Mayflower, Detectives Huston and Piccirilli recount what happened to Jenna and Ryan, and Max denies involvement. They let him continue working, though their suspicions about his connection to the deaths linger.

Max follows a guiding light from the main mirror and discovers a box containing Eleanor Reigns's ID, a missing employee. The light leads him to a missing-person flyer, and he contacts Eleanor's sister Elizabeth to learn more about her disappearance.

Max and Elizabeth uncover that someone deleted surveillance files on the night Eleanor disappeared, erasing critical evidence. Their investigation points to a deeper conspiracy tying Eleanor's fate to the Mayflower's management.

Max and Elizabeth learn Eleanor is dead after speaking with Henry Schow, the former guard whose mouth bears a reflection scar. Henry reveals that Keller raped and murdered Eleanor and then forced him to delete the surveillance files to cover up the crime. They discover Eleanor's body was buried in the Mayflower basement crawlspace two months earlier, during the building's groundbreaking party, incriminating Keller. Henry's confession marks a turning point in the investigation.

Eleanor's trapped spirit begins seeking revenge on those responsible for her death, manipulating reflections to influence events. She uses Henry's fear to propel her actions and makes contact with Max, threatening his father to coerce his help. Max slowly accepts that his near-death experience has given him the ability to see and interact with spirits.

Max and Elizabeth search the Mayflower basement for Eleanor's body and are pursued by Keller, who chases them after learning their knowledge. Max saves Elizabeth from a near-strangulation, and Eleanor appears again, pulling Keller into the main mirror and killing him. The mirror confirms Eleanor's victory and leaves Max and Elizabeth stunned.

At the police station, Henry confesses to his role in the cover-up and the detectives treat Keller as a fugitive for Jenna and Ryan's deaths. In the interrogation room's one-way mirror, the cracked reflection reveals Eleanor in its glass as she kills Henry, sealing the investigation with a grim finality.
