Gabriel escapes from the hospital, causing chaos and violence. He brutally kills several staff members in a locked room before being subdued and detained by Dr. Weaver and her team. Madison Mitchell lives in Seattle, experiencing her fourth pregnancy after multiple miscarriages. Her abusive husband Derek blames her for their losses, leading to a violent fight that results in Madison sustaining a head injury. She is emotionally devastated by the loss of her baby and Derek's death.

Madison watches footage that reveals her biological mother, Serena May, was a teenage rape victim who gave her up, and that Weaver's team experimented on her as a child, attempting to remove Gabriel. They hid her true origin, which explains his parasitic nature inside her head.
