Ending explained
What Happens at the End of MadS?
How MadS (2024) ends, in short: the final scene, the last twist and what it means for the characters. The full ending explained, with every spoiler, lives on its own page.
In the ending of MadS, it becomes clear that the woman who appeared in Romain’s car and caused chaos was part of a dangerous experiment. She was likely a lab-created entity with superhuman abilities, which made her nearly impossible to kill. The virus or infection she carried was transmitted through her blood, and Romain inadvertently spread it further after waking up and being contaminated by her blood. As the story unfolds, it’s shown that this infection turned the entire city into a frenzy of violence, with people transforming into bloodthirsty beings driven by an uncontrollable demonic side.
The disease’s spread was amplified by the chaos and drug use among Romain and his friends, who had been exposed to the entity, and their altered states seemed to accelerate or make their transformation more intense. Romain’s own transformation into a beast-like creature signifies how deeply the infection took hold. The climax reveals that the government intervened, sending commandos to contain the outbreak by capturing the infected. However, the chaos was widespread, and the infection had already become an epidemic, leading to citywide destruction.

In the final scenes, Julia finds her friend Anais attacking her and her own mother, having become infected as well. She escapes but is eventually captured by the military, who realize she is still uninfected. Before dying, a soldier instructs Julia to stay in the dark and avoid the infected blood—highlighting the virus’s dangerous nature. Julia is left devastated, clutching a weapon and overwhelmed with grief and fear, unsure of what the future holds. The ending emphasizes the tragic consequence of the chaos—ordinary people, caught up in the madness and contaminated by the experiment gone wrong—facing a bleak, uncertain fight for survival. The film leaves viewers with a haunting reminder that the horror was not just in the supernatural elements, but in the unstoppable and wide-reaching infection's devastating impact.
