In a haunting flashback, we witness a young Josh Lambert, portrayed by Patrick Wilson, struggling with the sinister presence of a blood-red-lipped woman—responsible for Elise’s demise at the conclusion of the previous film. Concerned for her son, Josh’s mother Lorraine reaches out to Elise and her ally Carl for assistance. They soon discover that Josh possesses the rare gift of astral projection during his slumber. As Elise conducts an unsettling interview with Josh, her inquiries provoke a reaction that leaves her visibly shaken. The duo engage in a game of ‘hot & cold’, leading to an eerie occurrence where Josh communicates with an unseen entity as a door mysteriously swings open.
Fast forward to the present day, Renai (Josh’s wife) is embroiled in an investigation with a police detective, who examines the circumstances surrounding Elise’s death. In an effort to escape their tumultuous past, the Lambert family moves in with Lorraine. However, their reprieve is short-lived as unsettling events commence once again. Renai hears disquieting noises—a piano playing and a baby’s chair rocking—while Lorraine confronts Dalton, who ominously warns her that someone lurks behind her. When Lorraine attempts to find an explanation, she’s trapped in the bathroom, only to encounter Josh, assuring her that all is well.
Tension mounts when Renai encounters a spectral woman, garbed in Victorian attire, who menaces her and the baby. As Renai desperately fights against the ominous forces, Lorraine becomes alarmed by the bizarre behavior exhibited by Josh. In search of answers, she visits Specs and Tucker, two paranormal investigators who assisted Elise, but they’re unable to help. However, upon lamenting Elise’s absence, they decide to contact Carl, the medium from 1986.
Carl returns to assist, utilizing word dice to commune with Elise from the other side. Through their questions, they uncover chilling revelations about Elise’s fate and the malevolent spirit behind it, leading them to the decrepit Our Lady of Angels Hospital. This site holds the key to solving the mysteries surrounding Josh’s possession and the dangers it poses.
Despite their efforts, disturbance ensues back home when Josh finds Renai unconscious. Under the influence of an unseen force, he begins to act strangely, prompting suspicions from Renai. As they delve deeper, they discover Josh’s startling connection to serial killer Parker Crane, also known as ‘The Black Bride.’ The horrifying truth unravels as they navigate the shadowy realms, leading them on a rescue mission for Josh within the Further.
In a harrowing showdown, Elise confronts the unyielding evil that has entangled their family. The group faces insurmountable odds, but through teamwork and resilience, they endeavor to reclaim Josh’s true self and banish the darkness.
