In 1990, Susan Alves is confined in a Mauritius mental asylum where she frenziedly scribbles demonic images. Her husband Joe visits and they discuss their daughter Pia. Overwhelmed by fear, Susan commits suicide in front of Joe.

In present-day Mumbai, Pia is married to horror director Vic, but their relationship remains strained. They navigate professional pressures while trying to keep their marriage intact.

After Pia's father suffers a heart attack, Pia and Vic travel to Mauritius and stay at Pia's childhood home. They learn Pia had a twin sister, Dia, and that Dia's favorite doll was buried with her.

That night, Pia experiences a nightmare of a girl's ghost and objects in the house move eerily. She also has a disturbing flashback to Dia's drowning, realizing Pia was not with Dia when she died. These events hint at a hidden danger in the house.

Dia's ghost suddenly drops from the ceiling onto Pia, and the next morning Pia seems different and unsettled. The family grows increasingly uneasy as the disturbances continue.

Tracy the dog is found dead, alarming everyone. Carol contacts Aditya, a parapsychologist, and asks him to look into the supernatural happenings surrounding Pia. Unsettling occurrences in Mauritius intensify the danger.

At Carol's funeral, Aditya senses that Pia's life is in danger and urges Vic to seek help. Vic confronts the ghost and investigates the attic, uncovering Dia's doll, a video cassette, Susan's diaries, and morbid paintings.

Vic visits the nearby mental asylum, where a janitor reveals that Susan was not insane but possessed by an evil spirit. The discovery shifts the investigation from mental illness to supernatural possession.

Vic and Aditya watch the video cassette showing Pia drowning Dia, but they suspect the clip is only part of the truth. Repeated viewings reveal that Susan was already possessed before Dia's death, and a third spirit appears in the mirror, implying Pia is not the killer.

Vic discovers that the women in Susan's family become possessed when pregnant, and a possessed Pia kills Joe. This confirms a curse rather than mere tragedy.

The narrative reveals that Susan was pregnant with triplets; Dia and Pia survived while the third child faced complications, and Joe chose to save Susan. The spirit of the third child then seeks revenge against the family.

Vic and Aditya attempt to destroy the spirit; Aditya is killed, and Vic traps the spirit in another realm. Seven months later, Pia gives birth to a baby girl whose eyes suddenly go blank, hinting the spirit has now inhabited the child.
