The film opens in 1989 with the Carmichael family grieving after Margaret Carmichael dies in a car crash. Her psychiatrist, Dr. Farrel, notes that more people are experiencing similar symptoms since the Carmichael Manor investigators' deaths. The dreams blur the lines between memory and omen.

Bobby, son of Troy Hopewell, contacts Vanessa to discuss what he knows, but the evil clown murders him before she can receive his message. His death adds to the thread of victims tied to the mansion's dark history.

Vanessa is informed of Bobby's death by Dr. Farrel, reinforcing the sense that a malevolent force is targeting people connected to Troy Hopewell. Alicia, a true crime author who worked with Bobby, teams up with Vanessa. Through Bobby's notes, they discover that Troy Hopewell and four friends caused Margaret's car crash. Over the years, several family members have vanished or died under strange circumstances, with red balls found near the bodies. Vanessa learns her tie to the death of Margaret likely stems from her estranged husband.

In the same wave of incidents, Tom is attacked and killed by the evil clown. The victims' pattern reinforces the idea of a supernatural force enforcing punishment. Vanessa's nightmare intensifies as she is drawn deeper into the investigation.

During Vanessa's visions, she learns that Donald is an associate of Andrew Tully and helped influence Patrick to aid in building the Abaddon Hotel and starting a cult. The plan aims for immortality through the occult. Donald acknowledges Vanessa as one of two children born to a woman Tully impregnated.

The living reality reveals a shocking truth as Vanessa chases a man she saw near Carmichael Manor and discovers he is her twin brother Mitchell, believed to be dead. The film ends abruptly on this revelation, leaving her future and the town's fate uncertain.
