A teen girl named Marie struggles with breathing difficulties as she lies in the back seat of a car, where a worried woman comforts her while a male driver rushes them to the hospital. Upon arrival, as the woman carries Marie inside, the man sternly warns her: > “Not a word.”
In a separate scene, we are introduced to a seemingly simple family setup comprising Don and his wife Renee, who are raising their two daughters, Amy and Sara. While the family appears to be normal during dinner, trouble brews as rebellious Sara sneaks out to join a bonfire, accompanied by her motorcycle-riding boyfriend, Chris. At the bonfire, Chris expresses his feelings for her through a heartfelt song, solidifying their young love.
When Sara returns home the next morning, chaos ensues as Don, filled with anger, confronts her, asserting that while she resides under his roof, she must adhere to his rules. Overhearing conversations about a plan to move out once she turns 18, Don creates a chilling scheme by converting their basement into a bomb shelter, preparing to trap Sara within its confines.
As Sara unwittingly helps her father move items to prepare the basement, he locks her inside, soundproofing the room to prevent her from escaping or being heard. Day 1 begins a grim series of events where Sara finds herself living like a caged animal, unable to escape her father’s wrath and abusive tendencies. Don claims her punishment is to help her earn privileges for her behavior, taking a horrifying turn as he continues his abusive actions towards her.
Renee, believing her daughter has chosen to run away, reaches out to the police. Unfortunately, their hands are tied as Sara is technically an adult at 18. As the days progress, Sara’s mental and physical state deteriorates, haunted by memories of Chris. The struggle escalates on Day 21, where Don coerces her into wearing a dress for her birthday, and after a series of disturbing exchanges, her pleas for freedom are met with anger and violence.
Fast forward through the harrowing Days 38 to 354, we witness Sara become a mother, giving birth to Marie without medical assistance. Four years later, both she and Marie are trapped in the basement, forming an unbreakable bond amidst their horrific reality. Sara consistently tries to make the best of their dire situation, even offering Don a semblance of normalcy, such as tea during his visits.
As the years roll on, Sara gives birth to two more children, Michael and Thomas. The children show signs of anxiety and confusion, symptomatic of their isolated upbringing. Meanwhile, Chris continues to seek information about Sara, slowly piecing together the truth while facing increasing difficulties from Don’s aggressive presence.
As the dire circumstances mount, Don’s facade begins to crumble under the weight of his own issues, and his relationship with Renee spirals out of control. A glimmer of hope arises when Day 20 brings a medical crisis for Marie, necessitating an emergency hospital visit. In a dramatic turn of events, as the doctors work tirelessly to save her, Sara seizes this moment to reveal the dark truth to a doctor, leading to Don’s arrest and the revelation of his reign of terror over his family.
This shocking conclusion unveils the horrific truth about Sara’s nearly two-decade captivity, where wrongdoing lurks in familial bonds. As the family attempts to piece together their shattered lives, Chris takes Sara out for a much-needed break, reminding her of the aspirations they once shared. Their reunion is bittersweet, showcasing the difficult road to recovery that lies ahead.
