The Ashram community, led by Aghoras, condemns Shankar and banishes him, misreading his phobia as a curse from Lord Shiva. He remains an amnesiac Aghora living with a crippling phobia of water. The expulsion marks the start of a perilous journey toward healing.

Sudama, a compassionate Ashramite, directs Shankar to seek help from Kedar Baba. He notes that Kedar Baba was last seen at the Kumbh Mela in Prayagraj. This sets Shankar on the road beyond the Ashram walls.

Shankar learns that Kedar Baba has died. A disciple hints at Maalipatram, a rare bioluminescent mushroom with medicinal power that might cure him. The mushroom is potent only once every 36 years, and he is given a 15-day deadline to find it.

Jahnavi, a doctor researching Maalipatram, joins Shankar on his quest. They travel toward the Himalayas to a cave at Dunagiri where the mushroom is believed to grow. Her purpose is therapeutic—aimed at healing mental trauma rather than mere cure.

In a remote Southern Indian village, Durga loses her Devadasi duties due to a terminal illness and longs to spend time with her daughter Uma. Uma initially resents her mother’s absence, but later understands the life-saving vow behind her mother’s choices. Their bond remains a central emotional thread.

At the clandestine CAT facility CT-333 is coerced into conversion therapy. Tara aims to force results through electroshock, while Dr. Bakshi pushes for lobotomy, ignoring the severe side effects and ethical boundaries. The facility becomes a crucible of exploitation and control.

An inmate who previously worked under Bakshi hatches an escape plan and aligns with CT-333. They dig an underground tunnel as alarms rise, seeking to break free from the illicit program. The escape becomes a pivotal moment in the facility’s dark operations.

Shankar experiences haunting dreams of CT-333 seeking help, while Jahnavi proceeds with the search and is soon trapped in a frozen lake. He rescues her, and she explains her broader aim: to use Maalipatram to heal mental trauma rooted in past events. This deepens their shared purpose.

Durga’s attempt to escape with Uma is thwarted by illness and the chieftain’s schemes. Uma is forced into becoming the village’s Devadasi, a brutal turn that highlights the personal costs of the village’s superstition. The mother's vow and tragedy underscore the film’s critique of ritual exploitation.

CT-333’s escape attempt collapses, and Bakshi escalates to a forced lobotomy as Tara’s efforts fail to derail the fate of the experiment. The facility’s machinery becomes an engine of destruction.

Kedar Baba rescues CT-333 from a wolf pack and delivers a revelation: CT-333 is Uma, and Uma, CT-333, and Shankar are the same person. The twist reframes the entire saga as a single journey of one soul across multiple identities. The uncomfortable truth forces a reckoning with past experiments.

Shankar, realizing the truth, confronts his fate and seeks Maalipatram one final time. He fights a pursuing lion, consumes the mushroom on the prescribed day, and regains his long-awaited peace. He wakes by a lake and meets Jahnavi alive; the healing culminates in Uma holding Durga, completing the film’s emotional arc.
