Tony Stark is a renowned weapons designer whose creations end up being used in conflicts abroad, including Afghanistan, where a mission goes wrong and he is seriously wounded by a bomb he helped design. A shrapnel piece lodges near his heart, and he is captured by Afghan terrorists. With the help of his ally and fellow captive Ho Yinsen, he builds a first Iron Man suit in order to escape, though Yinsen sacrifices himself in the escape.
In a separate strand, three men in a Texas slaughterhouse witness a disturbing transformation when one of them, nicknamed “Mallen,” is injected with an experimental drug. The procedure triggers a painful metamorphosis and leaves the others terrified enough to flee and lock the door behind him.
Back in New York, Stark lies dormant in a Coney Island garage until a routine interview reminder jolts him to action. During the interview, he confronts his own regrets: the world-longing dream of funding humanitarian progress through weapons sales has not yet materialized. This moment of reckoning convinces him that the Iron Man suit could be the key to a better future—for himself and for humanity. He cancels his schedule, dons the newest armor, and soars into the sky, embracing the possibility of a new path.
Meanwhile, the rapidly changing injected man in the Bastrop warehouse continues to evolve, his body now covered in a bizarre layer of scar tissue as he heals.
In Austin, at Futurepharm, Aldrich Killian records a confession after destroying the company’s records and admitting he has released the Extremis serum for a larger purpose. Maya Hansen, a scientist from years past, is alerted to these events and reaches out to Stark, invoking their shared history and the possibilities—and dangers—of Extremis.
The Texas slaughterhouse trio returns to find Mallen nearly recovered, still alive and capable of bursts of power that threaten anyone nearby. Stark arrives in Texas in the middle of a high-stakes boardroom teleconference, resisting calls for his resignation and arguing that Stark Industries does not need direct government funding thanks to a revolutionary new phone technology. He dispatches an urgent digital request—sending Killian’s hard drive to be hacked by a trusted colleague—and, to keep Maya on board, escorts her to meet an old mentor, Sal Kennedy, in San Diego.
As the FBI field office becomes the next target, Mallen’s powers erupt into a public crisis, with civilian casualties mounting as Maya reports the spread of Extremis to the media. Stark learns that Killian has supplied Extremis to local terrorists and realizes the scope of the threat. He rushes to Futurepharm to obtain the means to counter the danger, and uses his armor’s non-lethal repulsor capabilities to stop the truck-tank pursuit of Mallen, only to find that Mallen rebounds with even greater speed and strength.
In a brutal clash, Iron Man is pushed to the limits as Mallen fights through a shattered cityscape. Stark is forced to endure devastating injuries, and the police must help lift a burning car off him as his suit struggles to keep pace. The battle ends with Stark leveraging thermal energy from the flames to power his suit at a critical moment, then chasing Mallen to a confrontation that tests both their ideologies.
With the threat momentarily contained, Stark reveals his true identity to Maya during a tense private moment in the Futurepharm facility. He exposes her possible role as Killian’s accomplice, based on the double-lock security that required two keys and her intimate involvement with the Extremis project. Maya accepts the consequences, justifying Extremis by comparing it to weapons of mass destruction—and Stark responds with a quiet resolve to live with himself.
