Ike, Kane Hodder is a man haunted by a tragic accident: one drunken night out hunting ends with him accidentally shooting his pregnant wife Virginia Bryant in the eye. Since then, he is tormented by visions of his wife and their unborn son, pushing him toward atonement through a grisly ritual: to obtain replacement eyes. At Exit 33, he targets women who pull into his secluded gas station. He sabotages a pump so their car stalls a short distance away, then he drives up, subdues the woman, and drags her back to his dim, trophy-lined station. There, he gouges out her eyes, and preserves them, tucking them among the mounted hunting trophies in his den.

On a road trip to their fifth high school reunion, Angie, Antoinette Nikprelaj and Eva, Maria Hildreth (whose boyfriend is beheaded with a hacksaw) are captured by Ike. He severs Eva's eyes and watches as she bleeds out, while would-be heroes—Jerry Reid as Matt, a pair of hunters, and an off-duty officer—arrive, only to fall to Ike’s ruthless violence. Ike even loses an eye in a brutal confrontation with Matt, and though the attempts at rescue continue, Angie's eyes are eventually claimed for another trophy. In the final moments of the den’s grim stillness, Ike speaks to his imagined son about how the next hunting season should be just as fruitful as this one.
