Dr. Robert Morgan loses his wife, Virginia, and daughter, Kathy, to the plague. He makes the heart-wrenching decision to bury Virginia in secret, rather than take her to the public burn pit reserved for his daughter's remains.

After burying Virginia, she returns home in a monstrous form and attacks Morgan. This chilling encounter forces him to recognize the necessity of eliminating plague victims using wooden stakes.

By 1968, Morgan has developed a grim daily routine centered around survival. He marks off calendar days, arms himself, and hunts the vampires that threaten his existence in a desolate world.

Morgan speculates that he may have gained immunity to the plague when he was infected by a vampire bat during his military service in Panama. This points to a unique advantage amidst the widespread chaos.

One day, a dog wanders into Morgan's neighborhood, rekindling a flicker of hope for companionship. After initially failing to catch it, he finds the injured dog on his doorstep and decides to care for it, yearning for connection after years of isolation.

Despite his attempts to save the dog, Morgan discovers that it too is infected. Devastated, he prepares to bury the dog, further deepening his solitude and despair in a world that feels increasingly hopeless.

While burying the dog, Morgan spots a woman named Ruth. Initially frightened, she eventually returns to his home, igniting Morgan's suspicion and caution regarding her true nature.

After confessing to Morgan about her sensitive stomach caused by garlic, Ruth reveals her group of infected individuals who are receiving treatment to counter the plague. This new information rattles Morgan and complicates his understanding of the survivors.

Morgan, driven by desperation, bravely performs a blood transfusion on Ruth to cure her. This act of hope temporarily strengthens their bond and opens the door for a potential solution for the infected.

The fragile peace crumbles when Ruth's companions attack Morgan's home, forcing him to flee for his life. During the chaos, Ruth's group eliminates the remaining vampires, leaving Morgan feeling cornered and betrayed.

In a desperate attempt to escape, Morgan uses tear gas grenades from a police station against his pursuers. Ultimately, he finds himself trapped in a church, where a fatal spear thrust brings his struggle to a tragic conclusion.

As Morgan dies from his injuries, he defiantly denounces his attackers as 'freaks' and claims to be the last true man on Earth. His words resonate with despair and resistance as the last vestiges of his hope are extinguished.

In the aftermath of Morgan's death, Ruth hears a baby crying nearby, symbolizing a flicker of hope in a dark world. She tries to reassure the child, suggesting that perhaps there may still be a chance for humanity’s survival.
