In the rural Northern California mountain community of Paddock County, a nurse named Mirna Dobson is brutally murdered by a masked killer with an axe while driving through a car wash, setting a chilling tone for a string of grisly crimes that will rock the town.

One morning, Gerald accompanies his friend Richard Simmons, an exterminator, as they investigate a putrid smell emanating from the basement of a local tavern. There, they uncover the rotting corpse of Mary West, a missing barmaid, wedged in an attic crawlspace, a discovery that leads authorities to write off the death as a suicide and begin threading the case through a different lens.

Shortly after, Lillian Nebbs, the daughter of another tavern owner and a college-bound local, forms a budding connection with Gerald through their shared interest in technology. Frank McIntosh begins to lead the investigation, trying to thread together the disparate clues that keep appearing at the scene of each crime.

In a moment that tightens the web of clues, Lillian discovers a list within the computer that catalogues the three women who have been killed so far. When she asks Gerald about the list, he explains that he sometimes compiles data for amusement, a fact that fuels her growing unease and suspicions about who might be behind the killings. The tension intensifies during a windstorm when another local woman is attacked inside her own home; the killer, having previously left one of her hogs’ severed heads in her bed, returns to strike again, catching her in the privacy of her barn before delivering a fatal blow with the axe.

The following day, while out on the lake, Richard uncovers the severed head of a nurse from a local psychiatric hospital, a discovery that deepens the sense of a malevolent pattern taking shape across the town. That same night, Anna Bixby(/actor/may-heatherly), a leader of Lillian’s church choir, finds her own dog murdered in her house. When she goes to retrieve her shotgun, the killer, lurking in the pantry, nicks her fingers and then murders her, a brutal act that compounds the fear seeping into every corner of the community.

As the body count rises, Lillian opens up about a troubling fear: she has uncovered that her cousin, Charlie, was released from a mental hospital years earlier after a head injury she caused by pushing him from a swing. He becomes a potential suspect in her mind as she grapples with the possibility that someone she knows could be responsible for the carnage. The plot thickens when Lillian uses Gerald’s computer to pretend she’s researching a graduate program at the University of Portland, and Gerald discovers a surprising list of psychiatrists in the search results. That evening, Laura, Richard’s midlife artist wife, attends the Nebbs’ tavern and drinks heavily after learning she is bankrupt. She briefly exits the vehicle and rests against a tree; when she returns, she finds the masked killer in Christopher’s seat. The attacker pursues her into the woods and kills her, a brutal end that underscores the danger simmering just beneath the surface of life in Paddock County. The next morning, Richard learns that Laura never returned home and claims she emptied their bank account before vanishing. The bodies of Laura and Christopher are discovered later that evening, and at the crime scene, Officer McIntosh finds a tavern pin that leads the police to question Lillian Nebbs and her father.

Meanwhile, Lillian, who is home alone again, hears strange noises downstairs. She is confronted by Gerald, who insists that Charlie is not real and counters with medical records and a dossier of psychiatric confinement supposedly connected to her past. He also presents a list of all the victims—every local hospital worker who cared for Lillian or women who were romantically involved with her father—an unsettling montage that makes her feel as though she is being gaslit. The confrontation spirals into a tense struggle, and when the authorities arrive, McIntosh and his deputies shoot Gerald to death. In the aftermath, McIntosh consoles a shaken Lillian, who then smiles with a chilling recklessness over his shoulder, a final image that hints at the depths of the mystery and the darkness that still lingers in Paddock County.
