Milo has lived in a safe, cradle-like limbo world that feels more like protection than a real world. He is about 11 years old in that realm and is simply scared of the unknown. The story establishes that he has never experienced physical sensations, emotions, or the challenges of real life, making the prospect of leaving his comfortable existence terrifying.

Elmore is a soul who lived a flawed life, particularly abandoning his daughter Elizabeth. He is trapped in a limbo-like waiting area because he must prove himself worthy of heaven. The authorities explain he was offered a chance and chose to gamble with it, leaving him unable to move forward until he demonstrates genuine redemption.

If Milo does not choose to be born before midnight, the birth door closes and no more babies can be born anywhere in the world. The story shows this causing a global crisis with births becoming inexplicably few. The entire world's future literally depends on Milo's decision.

The story operates on a rule that babies must be born in order. Since Milo is next in line and he refuses to choose birth, no other infants can arrive until he makes his decision.
