Elizabeth Bridget Fonda and Kevin Campbell Scott live in a Manhattan loft above his glassblowing studio, eagerly awaiting the arrival of their first child. They rush to the hospital when Elizabeth believes labor has begun, only to be told by the obstetrician that she is actually experiencing Braxton Hicks contractions.

The reality is that in the world from which babies come, Milo Anton Yelchin is too scared to leave the safe place where he has lived, a place that feels more like a cradle than a world, even though he seems to be about 11 years old in that realm. Since babies must be born in order, no other infants will arrive until he makes a decision. The powers that govern birth are at a loss, and an old man named Elmore Albert Finney volunteers to show Milo that life is worth it. Elmore is trapped in a limbo-like state, aching to return to Earth because this afterlife limbo feels tedious, but he cannot move on until he proves himself worthy of Heaven.

Seeing a chance, Elmore bargains with the authorities: Milo must choose to be born in exchange for more time on Earth. To persuade the boy, he leans on Milo’s love of card games and tricks, letting the luck of the cards decide their fate. The moment is tense and strange, but the gamble is on. Elizabeth calls her mom, Lesley Ann Warren updating her about the strange turn of events, a reminder of the human connections left behind as worlds collide.

Elmore and Milo exit through a door that normally serves only as an entrance, and suddenly they are in the energetic and at times terrifying New York City. Elmore loves the city’s vibrancy, while Milo is overwhelmed by its scale and noise. Milo is briefly introduced to the wonders of food—deli sandwiches, ice cream, and the awes of Central Park—experiences that feel new and wonderful to him, though the moment is tempered by his growing awareness of his own bodily needs, which he has never had to manage before.

A call from above hints that Elmore should try to awaken Milo’s soul, but a museum visit seems to dampen Milo’s budding curiosity, and the day’s events ripple outward as the world watches a global crisis: births are inexplicably few, and Milo’s hesitation seems to have reverberations beyond their small circle.

Back at Kevin’s studio, Elizabeth cleans up after a pipe burst and discovers a bag full of Atlantic City paraphernalia that once belonged to her father. She impulsively heads to Atlantic City, where the thrill of gambling feels like a test of control and fate. Almost simultaneously Elmore leads Milo to the same city, drawn to the memory of his own love of gambling when he was alive.

The pressure thickens as the night nears: at midnight, the birth-door will close, and no more babies will be born. In Atlantic City, Milo comes to a piercing realization: Elizabeth is his mother, and Elmore is the father who abandoned Elizabeth, effectively making him Milo’s grandfather. This insight crystallizes Milo’s resolve to be born, even as the two men confront the consequences of their past.

But Milo’s strength wanes, and the path back to the birth door seems almost out of reach. Elmore, overcome with remorse for how he treated his family in life, decides to intervene in a selfless moment: he guides Milo back through the door, foregoing his own chance to stay on Earth. The powers that be do not deny the choice, but they remind Elmore that he was offered a chance—and chose to gamble with it.

In the end, Milo is born, and Elmore ascends from limbo toward heaven. The story closes on a note that blends tenderness with a quiet, mythic sense of consequence: a family comes together, a life begins, and the quiet, enduring pull of love helps even the most reluctant soul to take a step into the world.
