Chev Chelios regains consciousness after plummeting from a helicopter, only to discover he is in a makeshift hospital. Shockingly, doctors are in the process of removing his heart while a man named Johnny Vang looks on.

Upon awakening, Chev realizes that his heart has been placed in a locked red cooler and an artificial heart has been installed in his chest. This alarming turn of events sets the stage for his desperate struggle for survival.

Chev overhears the medics planning to harvest more of his organs, which triggers his instinct for survival. In a frenzied escape, he violently fights back against the doctors and successfully breaks free.

After escaping, Chev realizes he must keep the yellow battery pack connected to his artificial heart to survive. He learns from Doc Miles that he only has one hour to live if the external battery fails.

While searching for directions to find Johnny Vang, Chev is involved in a chaotic car crash that destroys his external battery. In a moment of desperation, he connects jumper cables to himself to continue his quest.

Upon reaching the Cypress Social Club, Chev confronts a brutal customer while trying to save a prostitute named Ria. This encounter ignites a new passion within Ria, further complicating Chev's mission.
