One fateful night in the quiet town of Evans City, Pennsylvania, dairy farmer Mitchell succumbs to madness. After tragically slashing his wife’s throat, he proceeds to wreck their home and ultimately set it ablaze, all while his terrified young children look on. This shocking event leads to the activation of the volunteer fire department, pulling David (Will MacMillan), a Vietnam veteran and local high school coach, away from a romantic interlude with his pregnant fiancée, Judy (Lane Carroll).
As a nurse working with Dr. Brookmyre (Will Disney), Judy is soon called to assist in the treatment of the traumatized Mitchell children. Upon arriving at the doctor’s office, she is bewildered to discover a man in a gas mask directing the care of the children. Meanwhile, the firemen travel to the Mitchell farm, but they are met with an unexpected sight: a battalion of soldiers.
At the doctor’s office, Major Ryder (Harry Spillman), overseeing the soldiers, commands his men to don protective gear and announces an emergency quarantine. With genuine concern for Judy and her unborn child, Dr. Brookmyre discreetly advises Judy to administer an antibiotic to both herself and David, urging her to leave and seek refuge with him away from potentially infected townsfolk.
As the soldiers begin to occupy the town, David, fearing for Judy’s safety, drives back with his friend Clank (Harold Wayne Jones), also a war veteran. Concurrently, Colonel Chris Peckem (Lloyd Holler) is summoned to take control of the situation in Evans City, while Ryder makes calls to Brubaker (W.L. Thunhurst Jr.) and other officials in Washington D.C. Unbeknownst to the townspeople, a plane carrying a deadly biological weapon had previously crashed, contaminating the local water supply with a virus that induces madness or death. In a desperate move, the military begins to strategize for a potential nuclear strike on the town.
Dr. Watts (Richard France), the head of the team behind the virus, arrives in Evans City and warns Peckem that an appropriate plan hasn’t been set in motion. He expresses his concern about the rush to deploy military interventions without adequate analysis or foresight. With feelings of self-importance, Brubaker and top officials hastily enforce martial law, confining the panicked population to the high school.
As chaos ensues, soldiers clad in protective suits invade homes, forcibly removing frightened children as fear grips the community. After Peckem’s arrival, he orders preparations to fortify the borders of the town. The mayor, also serving as the local lawyer, threatens legal action against the soldiers for overstepping civil liberties, leading to a tragic escalation when a police officer is shot dead.
As the insidious virus wreaks havoc on the inhabitants of Evans City, one farmer decides to resist the quarantine, opening fire on the soldiers while his daughter innocently plays the piano and his elderly wife attacks a soldier with knitting needles.
Meanwhile, David, Clank, and Judy find themselves detained by soldiers and thrown into a van with other victims, including Artie Bolman (Richard Liberty) and his daughter Kathy (Lynn Lowry). An unexpected brake slam allows the trio to escape, but they are faced with the chaos of a town spiraling out of control. In their bid for survival, the survivors see the brutal reality of the quarantine as soldiers kill a family and set their remains ablaze before the horrified eyes of their young son.
As their journey continues, the group finds temporary refuge in an abandoned house, where the mentally unstable begin to exhibit signs of the disease. When Kathy shows distressing signs of dementia, David and Clank take action against the circumstances tightening around them. Amidst the backdrop of escalating strife, Clank, in his deteriorating mental state, becomes increasingly volatile, leading to further violence.
