Footage confiscated by the U.S. government is suddenly released by an anonymous source, revealing the true horror behind a coastal town’s nightmare. The film follows the small Maryland town of Claridge, tucked on the Eastern Shore where a reliable water supply is everything. A nearby chicken farm, under intense scrutiny for dumping waste into the Chesapeake Bay, unwittingly sets off a chain of catastrophic events. As rookie reporter Donna Thompson covers a cheerful Fourth of July celebration, the mood shifts abruptly when dozens of residents fall violently ill, developing brutal lesions. Overwhelmed, Dr. Jack Abrams, the head of Atlantic Hospital, calls in the CDC, who initially suspects a viral outbreak. Yet the illness spreads with astonishing speed, and the town spirals into chaos as people begin dying within hours. Through the chaos, teenagers and adults alike report bizarre symptoms on FaceTime, including the disturbing sensation that bugs are crawling inside their bodies.

A hidden cause begins to emerge. A few months earlier, two oceanographers had found alarmingly high toxicity in the bay. They witnessed fish dying from the inside out and, after deeper investigation, identified the true culprit: enormous, mutated tongue-eating isopods. The bay water’s pollution traced back to a surge of excrement from steroid-fed chickens, and the resulting ecological imbalance allowed the isopods to proliferate unchecked. They prey on living hosts from within, leaving the water haunted by lifeless fish and a growing epidemic of lesions. The oceanographers’ attempts to alert authorities were ignored by Mayor John Stockman, who dismissed the warnings. Ultimately, a swarm of fully grown isopods destroys the oceanographers, their bodies later found as a grim reminder of the crisis.

Normal life is erased as a quarantine clamps down. The Talmet family—Alex, Stephanie, and their infant Andrew—sail into Claridge, seemingly unaware of the danger that has already taken root. The town is sealed off, with law enforcement shutting down cell towers, and the isopods have spread even to Stephanie’s parents. When two deputies, Jimson and Paul, respond to a scream, they hear a digitally enhanced audio recording of an infected family begging to be saved—and then killed. Consumed by the scene before him, Jimson euthanizes them and then murders Paul after an isopod bites him. The confrontation with Stockman and Sheriff Lee Roberts escalates rapidly; Jimson shoots Roberts and then takes his own life, while Stockman tries to escape in Roberts’ car, only to be killed in a fatal collision.

The mystery deepens as the authorities learn the true nature of the threat. The CDC discovers that Claridge’s water carries a significant radioactive signature. With the White House alerted, Abrams is told that no extra help will be provided and that he should evacuate. He refuses, choosing to stay and document the hospital’s mounting dead—Jennifer among them—before realizing that he is infected himself. In his final hours, he records the haunting pile of corpses that fills the hospital halls.

Journalists on the ground keep filming as the system fails. Donna and her cameraman, Jim Hoyt, persist in documenting the carnage even as her station’s blog is shut down by the FBI. Donna narrates that she fled Claridge and never spoke of the event again, while Jim later succumbs to the danger as well. The Talmet family returns to a town that is almost unrecognizable: most of Claridge is deserted and strewn with bodies. Desperate for help, they reach out to a friend via Skype, only to face the terrifying reality that Alex, who had previously swum in the bay, is infected and dies as the isopods crawl from his neck. Stephanie manages to escape with Andrew, while a still-living woman hides in a police car and pleads for aid—Stephanie’s impulsive act, striking the woman and causing her neck to snap, marks another brutal moment of the crisis.

Time passes, secrets surface, and the truth is laid bare. Years later, Donna releases the footage, revealing that the government ultimately killed the isopods by flooding the bay with chlorine. The official line framed the disaster as the result of unusually high water temperatures, and a handful of survivors, including Donna, were paid to remain silent. Stephanie, though still alive, declines to participate in the film. The movie closes on a chilling note: civilians continue to enjoy the bay’s waters, unaware that as much as 40% of the bay remains lifeless, a stark reminder that the cost of secrecy often comes at the price of public safety.
