As a result of relentless efforts aimed at eradicating the drug crisis in Philippine society, many barangays have been transformed into peaceful sanctuaries, free from the clutches of addiction. In this transformed landscape, authorities shift their focus to Biggie Chen (Arjo Atayde](/actor/arjo-atayde)), a notorious drug lord whose location remains a closely guarded secret. To extract information about Chen’s possible hideouts, Detectives Dela Cruz (Lao Rodriguez) and Alvarez (Noni Buencamino) employ stern interrogation tactics on a low-ranking drug dealer named Teban (Alex Calleja).

Their investigation eventually leads them to Barangay Gracia ni Maria in Tondo, Manila, where a meticulously plotted “buy-bust” operation is initiated to apprehend Chen. Meanwhile, rookie police officer Nina Manigan (Anne Curtis), still grappling with the traumatic loss of her entire former squad during a disastrous drug raid orchestrated by corrupt officials, joins an elite anti-narcotics unit within the Philippine Drug Enforcement Agency (PDEA). This newly formed unit is charged with dismantling Chen’s operations and restoring justice to the beleaguered streets.

In a daring move, Manigan’s squad sets out to execute a carefully crafted entrapment operation in Plaza Rajah Sulayman, using Teban as bait to lure Chen into their trap. However, when Chen fails to appear, the team divides into Alpha and Bravo units to comb through the slums of Gracia ni Maria. Within this complex network of alleys, Teban covertly arranges a meeting with Chongki (Levi Ignacio) to facilitate a rendezvous with Chen. Little do they know, their operation has been compromised, revealing a gruesome plot aimed at slaughtering PDEA officers.

What follows is a devastating bloodbath that decimates the Bravo team, leaving Detective Dela Cruz as its sole survivor. The Alpha team, under the leadership of Lacson (Victor Neri), finds itself ensnared in a dire confrontation against the ruthless slum dwellers and drug mafias. In this heart-pounding, adrenaline-infused struggle for survival, Manigan uncovers shocking truths about Dela Cruz, realizing he is complicit in the very illegal trade she has vowed to dismantle. With a heavy heart, she takes matters into her own hands, rendering justice upon Dela Cruz, marking a pivotal shift in her journey as a dedicated anti-narcotics warrior.

As tension escalates among the residents of Gracia, it reaches a boiling point with a catastrophic uprising against the dual forces of oppressive law enforcement and merciless drug lords. This chaotic maelstrom results in the deaths of Rico (Brandon Vera](/actor/brandon-vera)), Bernie (Victor Neri), Teban, Chongki, Solomon ([icky Pascua), Manok, and nearly 300 innocent civilians, their fates irrevocably intertwined with the surrounding chaos.

In the aftermath, Chen is finally captured by the authorities, and Manigan, the lone survivor of her squad, uncovers a shocking revelation: Alvarez is the true mastermind behind Chen’s illegal dealings. While Manigan endures confinement, she witnesses Alvarez engaging in a clandestine phone call about “loose ends.” The eerie calm before the storm shifts abruptly when Alvarez coldly eliminates Chen. Seizing the moment, Manigan struggles with Alvarez for his weapon, triumphing over him and his associates. In a desperate attempt to conceal her actions, she injures herself before planting the gun on Chen’s lifeless body and appropriating Alvarez’s phone as potential evidence.

As PDEA agents interrogate Manigan about the events that transpired, she coolly insists that Chen had put up a fierce fight. The film concludes with a news broadcast reporting merely 13 casualties from the encounter—a jarring contrast to the grim reality where hundreds of lifeless bodies lay strewn across the streets.
