Thomas, Kevin Hicks accompanies his girlfriend Marie, Lydie Denier, to a sprawling winter estate that feels more solemn than welcoming. The house sits like a mausoleum in the snow, and Thomas recalls that his mother Sheila, Marilyn Haskell, died there years before, a memory that lingers in every corridor and echoes in Marie’s questions about the family’s odd comforts and odd silences.

Andreas, Jan Rubeš, Thomas’s estranged father and a renowned neurosurgeon, arrives after a long hospital shift and finds the house already charged with unspoken tension. The moment he discovers Thomas in a bathtub with what looks like a suicide note, it’s revealed to be a prank—a painted wound on his neck that hardly disguises the real fear this father’s ailing heart cannot ignore.

That evening, the table is crowded with Thomas’s brother Jack, Steven Saylor, and Jack’s girlfriend Sharon, Lynne Adams, as Andreas hosts a meal that turns quickly unsettling. Thomas admits a raw, reckless longing that his father might be dead, lamenting that their earlier prank didn’t yield a fatal outcome. The party ends on a troubling note when Thomas and Marie witness Andreas in an intimate moment with Sharon, a sight that sows distrust and unease in the sprawling house.

The next morning, Marie visits the ailing Charles, the family patriarch, in the hospital, while Marie later tells Jack that Sharon had sex with his father. The afternoon brings a grim shock when Marie discovers the family cat mutilated in a closet, a sign that the household’s sinister undercurrents may have real consequences. Marie voices suspicions that Jack or Yuri, the quiet handyman, could be involved, but Andreas dismisses the claim, hinting that Jack stands to inherit Charles’s vast estate. He explains that Thomas’s bond with his mother left a fragile wound, a wound that never quite healed and now feeds the mansion’s darker turn.

As the day wears on, Marie grows suddenly tired, slipped into a dreamlike stupor by what seems like a drugged drink. She experiences a nightmare where Andreas performs a craniotomy on a terrified Sharon in the basement, a vision that compounds the day’s fear. After waking, she confronts Jack, who reveals that it was his notion to eliminate Andreas; he even proposes an alternative—join forces to kill Thomas so that Marie would inherit Charles’s fortune. Andreas storms in, subdues Marie, and restraints Jack to an operating chair in the basement, where he conducts a lobotomy on Jack, sealing a grim experiment in control and cruelty.

Marie awakens to a macabre tableau: Sheila lies in a hyperbaric chamber, eyes open but unresponsive, while upstairs Charles’s room seems to hold its own pulse of dread. Marie’s terror deepens as she flees and seeks help from Yuri in the guest house, only to be briefly recaptured. Yuri feigns a police call but instead summons Andreas. Marie escapes again, only to discover Sharon’s corpse in the greenhouse, culminating in her recapture and return to the basement, where Andreas readies another procedure.

Thomas arrives with a chilling revelation: he and his father have orchestrated a plan that would transplant the ailing Sheila’s brain into Marie’s healthy body, allowing Sheila to live on in a new vessel. As Andreas prepares for the requested surgery, Marie fights panic and betrayal, a single tear tracing her cheek as the operation begins. In a final, brutal turn, Andreas performs a craniotomy on Marie and removes her brain, the process unfolding as Sheila’s future—inside Marie—hangs in the balance and the mansion’s dark intent comes to a wrenching, unforgettable close.
