Cain Burgess is being escorted by police to a hospital after being caught. During the journey, he learns of his mother's death and sees her corpse, which deeply affects him. This event marks a turning point in his emotional state and sets the stage for his subsequent actions.

After witnessing his mother's death, Cain suddenly breaks free from the police escort. He escapes in a violent manner and seeks refuge, heading to a private bar. His escape signifies his rejection of the law and desire for revenge or answers.

Cain forcibly enters the bar by assaulting the bouncers. Once inside, he drinks and overhears patrons, including Tune, discussing the murder of a gangster named Rook. Cain's disruptive entrance initiates chaos in the bar.

Cain overhears Tune describing how an unknown assailant killed Rook. He intervenes with laughter, which provokes wrath from Tune, the bar patrons, and the barmaid Bez. His reactions stir further violence in the establishment.

Cain pulls out a double-barrelled shotgun and takes everyone in the bar hostage. He claims to be connected to Lincoln, the bar owner, which leads to a tense stand-off with the patrons and Lincoln's lieutenant Hyde arriving to recognize Cain as Lincoln's brother.

Cain blames Hyde and Lincoln for injuring him earlier and responds by shooting a patron in the kneecap. This violent act underscores his escalating aggression and mistrust of Lincoln's associates.

Cain narrates his past, revealing his time in HMP Belmarsh and his history as a skilled martial artist. He describes his fight for survival in prison and his transformation into a cold-blooded killer for self-preservation.

Cain seeks redemption by approaching Lincoln for financial help to start a legitimate business. Instead, he is drawn into Lincoln's criminal activities, including robbing a mark who retrieves a package from Lincoln's gang.

Cain and Lincoln engage in a deadly fight. Cain overpowers Lincoln and fatally stabs him after a brutal attack. He then spends his final moments before collapsing, having exacted his revenge, leaving behind the stolen wealth divided among his victims.
