Charlie B. Barkin is disillusioned by the afterlife and finds Heaven’s routine drains the spark from life. Itchy visits and echoes their old camaraderie, highlighting what they miss about living. The sense of dissatisfaction sets the stage for Charlie to take action when the moment arises.

Carface Caruthers steals Gabriel's Horn in a doomed escape attempt. He loses the horn somewhere over San Francisco as he tries to flee back to the mortal world. The theft triggers a heavenly mandate to recover the artifact.

Head angel Annabelle learns the horn is missing and tasks Charlie and Itchy with retrieving it. She grants them one miracle to aid their mission and urges them to act quickly. Charlie volunteers, drawing on his street-smart knowledge from the past.

Charlie and Itchy descend to Earth and arrive in San Francisco as ghosts. They quickly discover they cannot interact with the living world physically. The limitations complicate their plan to track down and recover the horn.

In a tavern, Charlie is enchanted by Sasha La Fleur, a beautiful Irish Setter. Carface appears in corporeal form thanks to a magical dog collar from Red, a crafty fortune-teller dog. Unknown to them, Red is actually a demonic cat with his own scheme regarding the horn.

Charlie and Itchy meet Sasha and an 8-year-old boy named David, who has run away to become a street magician. Charlie uses his miracle to let Sasha talk to David, causing David to believe Charlie is his guardian angel. A budding bond forms as Charlie tries to guide him.

Charlie watches the horn be carried into the SFPD station. He manages to recover it, but hesitates to return to Heaven and instead hides the horn in a lobster trap. The choice reveals Charlie’s inner conflict about staying or returning home.

David reveals his fear that his father and stepmother will love the new baby less. Charlie consoles him and promises to return him home, though he privately doubts his ability to keep that promise. Sasha offers emotional support as the trio clings to hope.

The collars that gave Charlie and Itchy their corporeal forms vanish, and they revert to ghosts once again. They are forced to operate without physical presence, complicating their mission and plans. The two must navigate their ethereal state to protect those they care about.

Carface kidnaps David and orders Charlie to bring Gabriel's horn to Alcatraz Island, delivering it to Red in exchange for David's life. Charlie, bound by his word, follows the dangerous terms of the deal. The stakes escalate as lives hang in the balance.

Red uses the horn to imprison Heaven's canine angels in Alcatraz and opens a portal linking the human world to Hell. The demonic cat intends to seize the horn and tilt the cosmic balance in his favor. Charlie fights to thwart Red's schemes and protect the angels.

After a pitched struggle, Charlie regains the horn and frees Heaven's angels, sending Red and Carface back to Hell. Charlie and Itchy are spirited away to Heaven, and Charlie returns the horn to Annabelle in exchange for a new life. The heavenly order is restored, but at a personal cost.

Charlie returns to San Francisco and reunites with Sasha and David. David reconciles with his father and stepmother, who welcome the new sense of family. Charlie and Sasha, now companions, are adopted by David's family as pets, while Itchy remains in Heaven with Annabelle. A hopeful new chapter begins for Charlie.
