A violent storm rages over Bangalore as a large tree branch crashes into an apartment, tearing the roof. The woman who inspects the outhouse is knocked unconscious and ends up in the hospital. The incident triggers the family's fear and sets the stage for what's to come.

In a separate town, Sanjana and her husband Kabir quarrel about not being able to celebrate her birthday together. A phone call reveals her mother has been in an accident, forcing them to rush to Bangalore. The looming emergency strains their relationship.

Upon arriving in Bangalore, Sanjana begins experiencing visions of her deceased conjoined twin, Anjana. Kabir dismisses the supernatural claims and arranges for her to see a former professor for spiritual healing. The visit marks the start of a supernatural struggle.

During the treatment, Sanjana tells the professor that Anjana envied Kabir's attention to Sanjana. She explains that the two sisters separated when Kabir planned to return from abroad, and Anjana died during the operation. Sanjana blames herself for Anjana's death, while the professor suggests her visions are the product of guilt.

The professor reassures Sanjana that her visions stem from subconscious guilt and not supernatural forces. He advises her to confront the past rather than chase phantoms. Sanjana leaves the session with some rationalization but little relief.

That night at her mother's place, an unseen entity drags Sanjana into the outhouse and leaves her unconscious. Kabir, awakened by noises, finds her in the dark and then she awakens possessed by Anjana. The possession marks a dangerous new phase in their ordeal.

After the possession takes hold, Anjana's fixation on Kabir grows stronger. The professor notes something is wrong and tells Kabir to keep a close eye on Sanjana. Tension escalates as the danger feels personal and intimate.

The caretaker suggests performing a ritual on Sanjana to rid the malevolent force. The ritual proceeds, but Anjana's power intensifies, prompting an exorcist to be summoned. The events blur the lines between superstition and menace.

During the kanjar ritual, Anjana reveals that she was murdered. She exits Sanjana's body and urges Kabir to uncover the truth about her death. The revelation deepens the mystery and the danger surrounding them.

Sanjana explains to Kabir that years earlier Anjana tried to kill her when Kabir phoned to say he was coming home. In self defense, Sanjana killed Anjana with a glass music box, and their mother kept the truth hidden to protect her daughter. The lie haunts them as they seek to make peace with what happened.

Following the professor's advice, they attempt to burn Anjana's remaining possession to release her soul. The plan brings them back to the outhouse where the fight for closure begins anew. The ritual underscores how guilt and memory drive the haunting.

They, along with the caretaker and Kabir's daughter-in-law, follow a musical cue to the outhouse and uncover Anjana's corpse beneath the floor. The caretaker and daughter-in-law are killed by Anjana's vengeful spirit as they fetch kerosene. The danger intensifies as the living race to end the haunting.

The professor arrives with kerosene but is possessed by Anjana and attacks Sanjana. Kabir battles the possessed professor and discovers a locket he had given Sanjana years ago on Anjana's corpse. Confronted, Sanjana reveals she is actually Anjana and that she murdered Sanjana to claim Kabir.

Kabir demands an end to the deception, and Sanjana pleads that she loves him. Anjana lunges at Kabir with scissors but is stopped by Sanjana's soul. The roof collapses in a fire, killing Anjana; the professor saves Kabir. In the final moment, Kabir sees Sanjana's soul smile before it vanishes in the blaze.
