Joseph wakes in his hotel room at 11:11 AM on November 7, 2011, and immediately feels the weight of a looming deadline. His manager Grant arrives, telling him his previous novel sold over five million copies and urging him to write again. Joseph begins drafting a painful confession about his wife's death and his loss of faith, noting that God may be dead or never alive.

At a support group on November 8, Sadie shares her story and offers Joseph encouragement. She tells him to speak up and hands him a notebook, hinting at a new outlet for his words.

After leaving the meeting, Joseph is involved in a car accident. He survives, while the other driver dies, and medics note no serious injuries. In the hospital, doctors reveal he is healthy, but Joseph notices his watch has stopped at 11:11.

That night, Joseph receives a call from his estranged brother Samuel, who is living in Spain with their father. He learns that their father, Richard Crone, is dying and may not have much time left.

On November 9, Joseph travels to Barcelona to visit his father. There he reunites with Samuel, who is in a wheelchair, and Samuel urges Joseph to let go of his resentment toward the church and stay the night.

In Samuel's study, Ana reveals a security video depicting a demon-like silhouette appearing at 11:11. Joseph studies the pattern of 11:11 and confesses he has been seeing the number repeatedly. He also encounters a diary titled El Libro de Ana, which Ana claims to be her gospel.

That night, Joseph learns that those who frequently see 11-11 are being Activated, and he suddenly finds demons attacking Samuel, whom he helps to rescue from strangulation.

On November 10, during a meeting of Samuel's congregation, Javier brandishes a gun and drops a camera; Joseph disarms him and the intruder flees.

The next stop is the cemetery where their mother Lauren is buried; Joseph notes that Samuel's birthday and Lauren's death both fall on November 11. Samuel urges Joseph not to search for meaning in every sign.

Joseph goes to a local shop to develop Javier's photos, and the staff promise they will be ready the next day. He also visits an occult bookstore, where the keeper reads a prophecy about November 11, 2011, predicting a sacrifice that will destroy faith and raise the Serpent.

On November 11, Sadie arrives in Barcelona and the trio are cornered in a maze by Javier, who again points a gun.

Joseph breaks into Javier's home and discovers Samuel's photos in a diary marked SACRIFICIO with 11-11-11. Javier confronts Joseph and shoots him.

Joseph calls Sadie to retrieve the developed photos as he races back to the house; Samuel is attacked by the demonic beings and is taken away at 11:02 PM. A sinkhole opens and Joseph crawls out as one of the beings readies a blade to kill Samuel; at the moment of 11:11, Joseph steps into its path and the beings disappear.

Sadie burns the photos; Joseph lies dying in the rain while Samuel reveals that the beings were angels and that he was the devil who had to be stopped. He hints that a new gospel will emerge from their books.

Sometime later, a large church is filled with Samuel's followers, each bearing a book titled The Book of Joseph, signaling the birth of a new religion.
