Four years before the main events, two brothers in Visakhapatnam—young Veerayya [Chiranjeevi] and Vikram Sagar [Ravi Teja]—form a close bond that is severed by a tragic accident caused by Veerayya’s vertigo. As adults, Veerayya runs an ice factory and secretly traffics contraband under the alias “Kaala,” while Vikram rises to ACP, marries Dr. Nithya [Shruti Haasan], and becomes a father to Vaishnavi. Behind the scenes, drug lord Michael Caesar [Prakash Raj] secretly uses Veerayya’s ice business to smuggle cocaine; when tainted ice kills 25 schoolchildren, Michael frames Veerayya and escapes to Malaysia. Vikram stages a police “encounter” to lure Michael back, but Michael’s men fatally stab Vikram, and his name is dishonorably besmirched posthumously. Nithya and Vaishnavi grieve, blaming Veerayya for both tragedies.
Now infamous and burdened by mounting legal fees, Veerayya is approached by suspended CI Seethapathi [Rajendra Prasad]. Seethapathi offers to clear Veerayya’s debts if he captures Solomon Caesar—an aggressive drug lord who has evaded RAW custody—in Malaysia and returns him to India. Veerayya accepts, accompanied by Seethapathi’s brother‑in‑law, Seethapathi’s ally [Vennela Kishore], and the two set out to Kuala Lumpur to hatch an abduction plan.
While scouting Solomon’s hotel, Veerayya unexpectedly encounters Nithya and Vaishnavi on holiday, prompting Nithya to pull her daughter away. A rogue tailing attempt by Seethapathi’s relative leads to his capture by Solomon’s men; Veerayya rescues him and tricks Solomon into believing he’s a drug buyer. At Solomon’s private party, RAW agents led by Atidhi—portrayed by [Shruti Haasan]—attempt a raid, but Veerayya, unaware they are government operatives, foils the attempt and earns Solomon’s gratitude. Promised a safe return to India, Veerayya unveils a plan: Solomon, upon discovering the ruse, tortures Athidhi on video while Michael watches. Veerayya then publicly kills Solomon in a crowded fair near the Batu Caves, forcing Michael to witness the act.
Back in India, Veerayya engineers a feud between Michael and partner Velappan Karuppan, crippling their operations and coercing Michael into cooperation. With Athidhi’s help, he overcomes his vertigo and escorts Michael to a court‑room confrontation that exonerates Veerayya and restores Vikram’s honor. Michael’s testimony clears Veerayya of past charges, paving the way for revenge against the mastermind behind the children’s deaths. To fulfill Vikram’s dying wish, Veerayya decapitates Michael in open court. Sentenced to prison, he attends Vikram’s state honors ceremony in custody, where Nithya and Vaishnavi finally forgive him, reuniting the fractured family.
