The movie begins with a man on an operating table, suffering from a severe abdominal wound. A surgeon cruelly tortures him by pouring salt into his wound, establishing an intense and disturbing tone for the film. This opening scene sets the stage for the themes of pain and suffering that pervade the story.

Four friends, Tayler, Tony, Kai, and Nick, are shown enjoying a vacation together, highlighting their close relationships. As they travel home, they stop at a gas station where Diane, an attractive stranger, asks for a ride after her truck runs out of gas. Nick agrees, but Tayler is suspicious of her intentions.

Diane returns to the car wearing a gas mask and administers nerve gas, rendering everyone unconscious. She then traps the friends in a house along with other captives, revealing her sinister plan. The kidnapped group awakens to find themselves locked inside with no immediate way out.

The captives discover gadgets inserted into their necks and a projector playing a message from a woman telling them they have 24 hours to escape. They are forced into a deadly game where inflicting harm on each other fills a meter, with the threat that failure will result in brain poison injection and death.

Greg, a reporter, tries to understand the situation and hopes to see his son again. The group beats him and inflicts injuries hoping to escape. However, they decide to limit torture to a maximum of 6% per person to avoid severe harm, showing their attempt to minimize casualties.

Tayler reveals she is pregnant, prompting Nick to volunteer to take her turn in the torture game. She secretly gives him medication pills to slow the meter, demonstrating her concern for her unborn child and her friends’ desperate situation.

Lisa tries to hide from the torturers but is dragged out by Sam, who continues with the brutal process. This event increases the tension and showcases the characters' struggle to evade violence within the confines of their captors' twisted game.

Greg recovers from injuries and learns that some members had considered leaving him behind to conserve strength. Despite the debate, Nick and Tony oppose abandonment. This moment highlights the moral dilemmas faced by the captives.

Tony, Tayler, and Lisa try to reach a 6% torture threshold by sticking their hands in boiling water. During this, Tara, still unconscious, is stabbed in the shoulder when she attempts to attack Tony for punching her, escalating the violence among the captives.

Tara, after regaining consciousness, pleads for relief but is subjected to brutal torture by Tony, who damages her vocal cords, damages her teeth, and cuts her arms. Her suffering pushes the meter to 80%, bringing the group closer to their grim goal.

With the meter reaching 100%, the projector announces their success in filling the bar and provides instructions to place vials into their gadgets for escape. Only Greg and Tayler remain, while Tony becomes trapped outside, heightening the chaos.

Sam kills Diane and Tony, revealing himself as the mastermind behind the twisted game. He locks Tayler away and discusses his belief that everyone deserves pain, contrasting his view with the other captives’ desperation to survive.

Nick kills Sam with a screwdriver, compensating for the poison with medication he finds. He then discovers that everyone else has been killed, leaving him devastated and alone, symbolizing the tragic outcome of the brutal ordeal.

Days later, Nick is shown eating at a restaurant, receiving medical treatment for his injuries. He notices Diane hitchhiking and, driven by rage, confronts her, ultimately knocking her out and abducting her, ending the story on a tense, unresolved note.
