Jamie and Maya, a young couple living in London, learn that Maya is pregnant and share a moment of joy. Shortly after, they are attacked by a gang of hoodlums, leaving them shaken and vulnerable. The assault becomes the catalyst for their decision to leave the city behind.

To escape the trauma, they move to a tranquil home in rural Ireland, inherited from Jamie's great-aunt Maeve after her death. They travel to the secluded property and begin settling in, hoping for a fresh start. The move signals a drastic shift from their previous urban life.

The house requires significant renovations, so they hire the local Whelan family as builders. The Whelans settle into the house and their presence begins to unsettled Jamie and Maya. A sense of unease grows as minor disturbances pop up around the property.

Niamh, an old friend of Jamie's aunt, warns that the Red Caps demand an offering of liver at a back garden door every night. Initially dismissive as superstition, Jamie and Maya eventually treat it as a wary superstition rather than a threat. The warning casts a chilling, persistent undertone over their nights at the house.

As they settle in, Jamie reveals Maeve had a daughter who disappeared when she was almost two, and her body was never found. Niamh recounts an old story that the child's disappearance was part of a deal with the Red Caps to save the father, who later died. Maya wonders if postpartum depression could explain the tragedy rather than a supernatural pact.

The Whelans prove themselves violent and disturbed: Aisling and Killian steal minor items from the house, and Eoin is beaten by Daddy Whelan. The atmosphere at the house grows oppressive as the family's cruelty escalates. Jamie and Maya realize danger is closer than they thought.

Maya is lured into the wood by a dog and discovers a stone shrine to the Red Caps. On returning, she encounters Eoin, who speaks of the abuse he endures, and when she offers him comfort he attempts to assault her. The Red Caps then drag Eoin away, confirming the supernatural danger in the forest.

That night, Jamie confronts Aisling and Killian in the pub about their brother's fate. Meanwhile Maya is visited by a Red Cap who presents her with Eoin's head in a plastic bag, proving the Red Caps are real. The revelation hardens the couple's resolve to uncover the truth.

Driven to desperation, Maya escapes into the woods to seek help from the Red Caps, promising to give them anything in return. The Red Caps respond, descend on the house, and kill all three Whelan parents. Jamie creates a distraction at the front door as Maya flees.

After the violent confrontation, Maya gives birth. Once the baby has been checked at the hospital and they return to the house, the Red Caps take the infant as payment for their aid. The parents are left to confront the supernatural bargain they've entered.

Maya follows the Red Caps to a shrine in the forest and discovers an elderly woman who she realizes is the lost daughter of Jamie's aunt, kept by the Red Caps. Maya refuses to let them keep the girl and kills one Red Cap, then crushes the old woman's skull with her bare hands. The Red Caps suddenly revere her as the new figure in their order.

Maya and her daughter return to the house as the Red Caps reverence her. Jamie watches in horror as Maya is baptized with the blood and skull of the dead woman, proclaimed the new Mother Redcap.
