Joyce, a caregiver in an elderly home, ends her relationship with Roman, a possessive boyfriend who nonetheless asks for one final favor—to drive him home in his car. The ride ends in tragedy: the crash kills [Roman] and leaves Joyce badly injured.

Alex, a call center agent who harbors feelings for Joyce, offers quiet support as she recovers. In the days that follow, Joyce begins receiving messages from Roman’s number, initially dismissed as a cruel prank. But the communications grow darker: they show Joyce what she’s up to, and then, at exactly 3:29 am, images of people she knows dying, forcing a creeping dread to take hold.

Aling Kuring, a restaurant owner who becomes entwined in the terror, receives a call from Roman. Thinking it’s a prank, she answers, and her head shakes violently as she dies. Lilia, Joyce’s patient, comes under Joyce’s watch after a text shows Lilia’s death. Before 3:29 am, Joyce falls asleep; the phone rings, Lilia answers, and she dies in the same jarring manner.

Joyce realizes that Roman is targeting those who did not support their relationship. She seeks help from Dante, a local anting-anting vendor and witch doctor, who asks if Roman had done occult acts or asked for possessions. Joyce breaks into Roman’s room but is caught by Edith, Roman’s mother, who blames Joyce for her son’s death. Then a new message arrives: a picture of Joyce’s best friend Ida. Joyce pleads with Ida to apologize to Roman for not supporting them, hoping to calm the spirit; instead, Ida defies Roman even from his grave.

Joyce is arrested for trespassing and detained for the night at Edith’s request. She bribes a policeman with a cellphone to escape and seeks help to protect Ida. Joyce reaches Ida’s home and waits for the police to arrive, but their chief sends them to a nearby accident. In the ensuing panic, Joyce leaves Ida alone in her room, and she is killed by Roman.

Joyce later acquires some of Roman’s possessions — a bloodstained phone and a paper bearing a photo of Roman with Joyce surrounded by incantations — and hands them to Dante. It’s revealed that, before the crash, Roman forced Joyce to perform a pact by letting drops of their blood fall on Roman’s phone, triggering the deadly struggle.

As Dante analyzes the other object, Joyce receives a warning that Roman’s spirit stalks her through her shadow. The haunting escalates when Roman takes over Joyce’s body and compels her to ride with [Alex]. Roman knocks them unconscious and seizes the wheel. Dante, driving in another car, tries to warn Joyce via text, but his vehicle collides with theirs. His car plunges off a cliff; Joyce’s car crashes into a post. Joyce fights to save Dante, but it’s too late. Dante manages to type one last warning that Roman will take over [Alex].

[Alex] emerges from the wreckage as lightning streaks across the sky, and Joyce discovers that Roman has indeed possessed him, stepping closer as the truth of the possession becomes inescapable.
