The film begins with the ending credits of The Human Centipede (First Sequence) rolling on a portable DVD player, revealing that a man stationed in a parking garage toll booth is captivated by the film. This man, Martin Lomax (Laurence R. Harvey), is depicted as an asthmatic, overweight, and mentally disturbed British man. Living in a cramped apartment with his verbally abusive mother, Vivien Bridson (Vivien Bridson), Martin works as a security guard in an underground garage where he becomes fascinated with the wealthy patrons using the facility.
With a haunting backstory, it is suggested that Martin was a victim of sexual abuse at the hands of his imprisoned father, a revelation that is troublingly confirmed through disturbing flashbacks. He becomes increasingly obsessed with the gruesome narrative of The Human Centipede, often indulging in graphic scenes from the film and even engages in a shocking act of self-gratification while continuing to live with his mother.
In a drastic turn of events, when Martin’s mother discovers and destroys his cherished scrapbook filled with memorabilia from the movie, he reacts violently, murdering her and setting her lifeless body at the kitchen table. This grotesque act ignites his ambition to replicate the horrific experiment he has fixated on from the film. Armed with household tools and a grim determination, he sets up a dingy warehouse to create not just a trio but a chilling twelve-person “human centipede.”
To execute his twisted plan, Martin, in a delusional state, reaches out to the actors from the previous film, masquerading as a casting agent for Quentin Tarantino. Only Ashlynn Yennie (Ashlynn Yennie), portraying herself, takes the bait and arrives in London. The film’s first hour graphically details Martin’s horrific actions as he hunts down and abducts several people, including a neighbor, a rental agent, and a couple expecting a child.
In a gruesome process, Martin incapacitates his victims by severing their tendons and violently extracting their teeth to prevent them from collapsing under the weight of their despair. He gruesomely violates the anatomy of his victims, using a makeshift method with a staple gun to connect their bodies in a horrific experiment. Unflinchingly, Martin chooses Yennie to lead the centipede, sparing her from the gruesome fate of consuming feces from the others.
As the experiment unfolds, Martin grows increasingly aroused by the suffering and desperation of his creation. He administers excessive laxatives, leading to chaotic scenes of bodily function and further torment. When a victim succumbs to their injuries, Martin’s breakdown, rife with tears, hints at the fragility of his mental state.
The narrative takes a harrowing turn when the pregnant woman, initially thought to be dead, awakens amidst chaos and makes a desperate escape. In a dramatic climax, her labor results in a tragic birth as she crashes the car, a scene invoking Martin’s rage as he futilely chases her.
The horror escalates as one victim escapes the confines of the centipede, a chain reaction that leads Martin to avenge his failures by shooting and then slitting the throats of his remaining victims, except for Yennie, who fights back with surprising ferocity. In a chaotic struggle that melds horror with surreal elements, both Martin and Yennie endure a visceral showdown culminating in violence and a potential twist of fate.
The film closes with Martin’s unsettling visage, watching the original Human Centipede once more, while unsettling echoes of a baby crying suggest the cycle of horror may not yet be over.
