April O'Neil, a reporter for Channel 6 news, starts her investigation into the Foot Clan, a gang that is wreaking havoc in New York City. She questions a dock worker about suspicious activities and stolen chemicals, setting the stage for her pursuit of the truth behind the gang's criminal operations.

Later that night, April witnesses the Foot Clan unloading cargo at the docks. As she attempts to capture footage of the illegal activity, a mysterious figure intervenes and defeats multiple Foot Soldiers, leaving April both frightened and intrigued by this vigilante.

In response to the escalating crime, New York City signs a contract with Eric Sacks to provide advanced security systems. Sacks shares his past with the Foot Clan, revealing the tragic events that resulted from their attack on his lab, which involved April’s father.

The Foot Clan, now aware of the vigilante in the city, begins terrorizing residents to force him out into the open. They execute a hostage situation in a subway station, hoping to uncover the identity of the vigilante.

During the Foot Clan's attack on the subway, April finds herself taken hostage. As chaos ensues, the vigilantes, who are the Teenage Mutant Ninja Turtles, arrive to rescue the captives, showcasing their martial arts skills against Foot Soldiers.

After witnessing the turtles defeat the Foot Clan, April manages to take their picture but faints from shock. Upon regaining consciousness, she discovers that the turtles have deleted her photo and are now wary of her, warning her to keep their existence a secret.

Back at home, April uncovers documents related to Project Renaissance, her father's research project that linked him to the turtles. Through her investigation, she starts to realize that the turtles she once saved are indeed the same ones she encountered in the city.

April visits Eric Sacks to discuss her findings about the turtles. Sacks reveals that the mutagen used in his lab years ago resulted in the turtles’ transformation into humanoids, leading April to understand her father's tragic connection to the Foot Clan.

Shredder learns of April’s connection to the turtles through Sacks and decides to use her to lure them out. Meanwhile, the Turtles are punished by Splinter for revealing their existence and are tasked to bring April to safety.

Splinter tells April about her past connection with the turtles, revealing that she saved them from a fire years ago. He explains how they have experienced accelerated growth and intelligence due to the mutagen, deepening April's bond with them.

Conspiring with Sacks, Shredder formulates a plot to unleash a deadly virus across New York City, which he plans to control by marketing the mutagen as a cure. Sacks reveals his motivation tied to personal vengeance against April’s father.

A climactic battle occurs in the sewer when Shredder and the Foot Bandits confront the Turtles and Splinter. During the fight, Shredder captures three of the Turtles and leaves Splinter severely wounded, escalating the stakes for April and Raphael.

Determined to save her friends, April teams up with Raphael to rescue the other Turtles from Sacks' laboratory. They manage to infiltrate the facility, leading to intense confrontations with Shredder's forces.

The Turtles confront Shredder on the rooftop of Sacks' building just as the toxin is about to be released. April finds the mutagen and assists in the battle, helping the Turtles to finally defeat Shredder.

Following their victory, as Shredder is captured by the authorities, April politely declines a ride with the Turtles. The film concludes with Michelangelo serenading her, highlighting the bond they have formed throughout their ordeal.
