Damian Hale, a billionaire from New York, is diagnosed with terminal cancer. He learns of a radical procedure called shedding, which transfers consciousness into a younger body. After meeting Professor Albright, he agrees and plans to stage his own public death to facilitate the transfer.

Albright completes the transfer, implanting Damian's consciousness into a younger body. Damian awakens in New Orleans under the alias Edward Kidner and starts a new life while concealing his true identity. He receives medication to manage the vivid hallucinations that accompany the procedure.

Damian, living as Edward, becomes friends with his neighbor Anton. He soon forgets to take his medicine, and begins seeing a woman and child in hallucinations. When he questions Albright about the visions, Albright dismisses them but inadvertently reveals details Damian had not shared.

Albright arranges a vacation for Damian in Hawaii, hoping the change of scenery will ease the transition. Damian experiences the memories as real, and identifies a landmark from his visions. He follows the clue to a farmhouse near St. Louis.

Damian arrives at the farmhouse and discovers Madeline, who treats him as her husband Mark. He learns that Mark may have sold his own body to Albright to pay for their daughter Anna's life-saving treatment. Damian plays along as Mark while coming to terms with the real possibility of his own past.

Albright's men attack the farmhouse; Damian fatally wounds Anton and kills his accomplices, then escapes with Madeline to pick up Anna from school. The danger of the shedding operation becomes personal as Damian confronts the consequences of his choice. They race to the school to ensure Anna's safety.

At a motel, Damian researches shedding and discovers Dr. Francis Jensen as the pioneer in transhumanism. He notices a shared tic between Jensen and Albright, and suspects that Jensen has shed his consciousness into Albright's body. The mystery deepens as Damian pieces together the conspiracy.

Damian locates Jensen's wife Phyllis in a nursing home but she has Alzheimer's. He lures Jensen (now Albright) to the facility and reveals that the pills are meant to erase the original personalities of the shed bodies, warning that without them Damian's original self may die and Mark's will return.

Jensen's men arrive; Damian is nearly overpowered but Madeline wounds the attacker, who proves to be Anton in a new body. Anton claims he has shed multiple times; Damian confiscates Anton's pills to prevent further re-embodiment.

Damian reveals the full story to his old friend Martin O'Neill and convinces him to help Madeline and Anna escape to the Caribbean. Martin becomes wary after learning the truth that Tony also shed and that Jensen's men are coming.

Damian stops taking his medicine to access Mark's memories and locates Jensen's lab in an abandoned warehouse. Jensen captures him and begins to shed Anton into Mark's body. Damian hides a bullet casing in his mouth, triggering a malfunction that destroys Anton's consciousness.

Masquerading as Anton, Damian rescues Madeline and Anna. Martin reverse-engineers the pills and provides Damian with a working formula, allowing Damian to burn Jensen's lab and kill Jensen with a flamethrower.

Damian visits his estranged daughter Claire and reconciles by leaving a letter. He travels to the Caribbean with Madeline and Anna, stops taking the medicine, and the original Mark begins to re-emerge. The movie ends with Mark reuniting with his family.
