In 1991, detective Joe Marshall plans to settle down with his girlfriend Jennifer after taking down Katana. Their peaceful future is shattered when a park encounter ends with Jennifer's murder by a young boy. Joe is left to confront the loss and the vow of vengeance that will define his later actions.

Twenty-five years after Joe's victory, Katana boss Fuj Fujiyama rebuilds the syndicate into a powerful multinational network. He informs his inner circle that the conflict with Shinjuku and Ginza has awakened Joe and must be defeated to secure Katana's future. The threat from Joe sets a new war in motion.

A senator is killed by Hera, a Katana agent, in a shocking political assassination. The murder demonstrates Katana's reach into politics and heightens the stakes of the clan war. The act sets off a chain of events that pull Joe back into the fray.

Katana launches a brutal assault on the Shinjuku at a nightclub, decimating their presence and escalating hostilities. The massacre reinforces the sense that Katana will stop at nothing to consolidate power. The event draws Joe out of exile and into a renewed conflict.

Joe is tracked to a secret compound 400 miles from Los Angeles, where he reunites with his old partner Frank Washington. He is reluctantly persuaded to reteam as they reopen the case seeking vengeance against the clans. The reunion marks his return to active duty.

Following his reunion, Joe is reinstated as a detective and begins surveillance on the warring Katana and Ginza clans. At the same time, he starts a tentative relationship with Milena Roberts, who bears a striking resemblance to his late Jennifer. The double pull of duty and romance deepens his internal conflict.

During a surveillance mission, Joe goes to a nightclub and encounters Milena, who greets him as if she were Jennifer. A firefight erupts between Katana and Ginza, and Frank warns Joe that Milena is not who she seems. The scene hints at Milena's true allegiance.

The next day, Frank is suspended for failing to stop the war. The case is handed to FBI Agent Carter, who is secretly Ginza's new leader, shifting the investigative balance and complicating Joe's pursuit of vengeance.

Despite the shifting leadership, Joe and Frank decide to operate outside the law to wipe out both clans at The Complex, Katana's hidden base. Their plan aims to deliver a decisive blow and end the escalating war.

Joe storms The Complex and engages the Ginza clan in a brutal confrontation. The siege fractures the base as guns blaze and swords clash, with key players racing to assert control.

During the combat, Carter is shot by Mola Ram, leader of the Yick Lung clan. Joe defeats Fuj Fujiyama and his henchwomen, then confronts Doggé in a sword duel. Linton Kitano, the Shinjuku's new leader, is quickly defeated and confesses murdering Jennifer twenty-five years earlier.

Milena intervenes to reveal she is Linton's sister and that she and Linton used Joe to infiltrate Katana and help Shinjuku become the country's sole clan. The revelation shatters Joe's trust and casts doubt on his mission.

Disillusioned, Joe leaves The Complex and parts ways with Frank and Detective Higgins. He steps away from the immediate conflict, contemplating the future and the potential for an uneasy alliance against a common foe.

Lauren Kimura, a former Katana henchwoman, approaches Joe with an offer to join forces, signaling a possible uneasy alliance for what comes next. The ending hints at shifting loyalties and future plans beyond the immediate war.
