What Happens in Penguin: Full Plot Summary

The whole story of Penguin (2020) explained, from the opening scene to the final frame: every key event, every twist and what it all adds up to. Read it as a quick recap before the sequel, to check a scene you missed, or to finally understand what really happened.

A sinister-looking stranger in a Charlie Chaplin mask strides toward a lake, carrying a bloody bag, as if echoing a dark ritual. At the center of this looming danger is Rhythm Rhythm, a pregnant young woman living in the scenic hills of Kodaikanal, who is seven months along with her husband Gautham. During a routine hospital follow-up, Rhythm’s mind slips back, and the present moment dissolves into a web of memories that reveal a past steeped in fear, guilt, and a shattering mystery.

Six years earlier, Rhythm was married to Raghu and they welcomed a son named Ajay. One ordinary day, Rhythm reads a children’s story about a missing penguin as Raghu breaks the news that Bhavana’s father has died. The couple visits Bhavana’s house with Ajay, where they encounter Rhythm’s friend Abi and her husband Kathir. Inside, Bhavana lies dazed and unresponsive. Rhythm tries to soothe her, but Bhavana remains uncommunicative. The mood shifts when Raghu accuses Rhythm of mishandling Bhavana’s distress and the family discovers that Ajay has vanished. A frantic search ensues, and it’s Kathir who finally locates Ajay outside the house, but the shadow of the disappearance lingers.

Back in the present, Rhythm sits in the clinic’s waiting room, attempting to keep calm as a nurse calls her in. She plays peek-a-boo with a young girl, only to realize the child vanished when she closes her eyes. A sudden swarm of insects on the ceiling triggers a gut-wrenching flashback, and Rhythm collapses. The doctor warns her that she is paralyzed by a fear of bugs and urges her to forget the past and avoid the lake. Yet the pull of the lake—the memory of Ajay and what happened to him—pulls Rhythm toward the edge once more.

The routine appointment becomes a premonition of danger when a routine day near the lake turns into an alarm. Rhythm is told Ajay’s school friends heard a whistle from the forest, and Ajay wandered off following it. Pavi, one of Ajay’s companions, mentions a man in a Charlie Chaplin mask seen in the woods. Rhythm’s worry escalates as police are sent to comb the lake region. In her grief, Rhythm slips into a daze, and the search team uncovers a chilling clue: a bouquet of human intestines with a Rest in Peace card. The sight sends Rhythm into shock, and the insects’ bite into her fear becomes a clear symbol of what her life has become. As time passes, Ajay’s bloodstained clothes are found in the forest, and the investigators grow convinced that Ajay is dead.

In the present, Rhythm’s memories flicker between happier times with Ajay and a new terror: a mysterious killer in a Chaplin mask stalking the town. She glimpses an injured man fleeing from the same masked figure and learns that he saw a missing child in the killer’s grip. The killer seems to spot Rhythm but lets her escape, and she clings to the belief that Ajay is the missing child—until a second missing girl named Anjana complicates the picture. Rhythm keeps searching, driven by love and fear, even as the police struggle to connect the clues.

At a carnival, Rhythm loses track of Ajay again, and Raghu reappears to help look for him. Gautham cannot be reached, and Ajay’s fate remains uncertain. A break occurs when a kidnapper breaks into Rhythm’s home and uses sign language to communicate with Ajay. Rhythm fights to stop the intruder, but the kidnapper escapes.

Ajay’s doctor, a man named David, suggests a speech therapist to help Ajay recover. On the way to the therapy session, Rhythm’s dog Cyrus discovers human remains in a dumpster and leads her to David’s house. Suspecting David, Rhythm uses a clever ruse to force him to leave his home, and a hidden room reveals Anjana and Ajay in danger. Rhythm rescues them and gets David arrested. In a revealing conversation with David, Rhythm learns that he has abducted children for their organs and that the bouquet’s arrival was a calculated distraction. He also hints that the kidnapper is close to Rhythm and urges her to rethink the situation from a new angle.

Rhythm’s suspicion initially settles on Raghu, given Ajay’s unusual reactions around him. She hurries to the hospital only to discover that Gautham has been attacked. A gloved clue hints that the kidnapper is female, pointing Rhythm toward a truth she had not yet considered. The trail leads her to a house where the kidnapper finally unveils herself: Bhavana. The revelation is brutal and transformative. Bhavana explains that, as children, Rhythm received the spotlight while Bhavana was left behind, and over the years she created a separate identity to cope with that pain. She recounts Ajay’s kidnapping as a day-long ordeal that stretched into years, and Abi’s husband Kathir’s fatal confrontation with Bhavana becomes a key turning point when Ajay’s safety was compromised.

Bhavana’s confession leaves Rhythm with a choice: save her unborn child or Ajay. A poisoned drink is offered, and Ajay is given the same deadly dose. Bhavana hopes Rhythm will feel the unbearable weight of her suffering, but Rhythm refuses to abandon Ajay. In a desperate moment, Bhavana shoots herself, and Rhythm, aided by Cyrus, discovers the underground room where Ajay is held and Kathir’s decomposing body. Rhythm frees Ajay, but the poison’s effects begin to take hold. Ajay, speaking for the first time, calls out to his mother as the two race to safety.

Rhythm ends up in the hospital, where tests show that both she and her unborn baby are safe, though weakened. Ajay resumes his playful self, a sign that life can begin to heal from such wounds, even after so much darkness.

- The tale collapses the distance between fear and hope, showing how a community’s hidden fractures can fester into danger while also illustrating the power of steadfast love to pierce through the deepest shadows. Rhythm’s journey—from a quiet life in Kodaikanal to a terrifying revelation—feels both intimate and sweeping, a testament to resilience in the face of a chilling predator and a past that refuses to stay buried.

Penguin Plot Explained: Your Questions Answered

The questions people ask most about Penguin (2020): what happens in the confusing scenes, why the characters do what they do, the twists, hidden details and what the ending means. Each answer contains spoilers.

Q1

Why did Bhavana kidnap Rhythm's son Ajay?

Bhavana kidnapped Ajay because of deep-seated jealousy toward Rhythm dating back to their childhood. Bhavana reveals that while they were growing up, Rhythm always received all the attention and spotlight while Bhavana was ignored and sidelined. This accumulated resentment drove Bhavana to create a separate identity and ultimately kidnap Rhythm's child as an act of psychological revenge.

Foreshadowing: Earlier scenes show Bhavana lying dazed and unresponsive when Rhythm visits her house, hinting at her disturbed mental state.

Q2

Who was the Charlie Chaplin masked killer?

The Charlie Chaplin masked figure was Bhavana herself, using the disguise to stalk the town and create an atmosphere of terror. She deliberately wore the mask to throw off suspicion and make it appear as if a serial killer was on the loose, when in fact she was the one behind both the original kidnapping years ago and the current crimes.

Foreshadowing: Pavi mentions seeing a man in a Charlie Chaplin mask in the woods, but the killer is ultimately revealed to be female.

  • charlie chaplin mask
  • killer
  • bhavana

Q3

What happened to Ajay during the six years he was missing?

Ajay was held captive in an underground room beneath Bhavana's house throughout the six years since his disappearance. He was kept alive but unable to speak due to the trauma. David the doctor was also involved, abducting children for organ harvesting, but he did not originally take Ajay - Bhavana was the mastermind holding him captive.

Foreshadowing: Rhythm's flashbacks show Ajay's bloodstained clothes being found, leading everyone to believe he was dead.

  • underground room
  • six years

Q4

Why did Bhavana target Rhythm specifically?

Bhavana targeted Rhythm because of the psychological trauma from their childhood, where Rhythm received all the family's attention while Bhavana felt invisible. This evolved into a twisted plan to make Rhythm suffer by taking away what she loved most - her child. Bhavana wanted Rhythm to experience the same pain of abandonment and loss that she felt throughout her life.

Foreshadowing: The opening scenes establish Rhythm as a mother who lost her son, setting up the emotional core of Bhavana's revenge.

  • bhavana
  • rhythm
  • revenge
  • childhood jealousy
  • psychological
Q5

What was David the doctor's role in the kidnapping?

David the doctor was a separate kidnapper who was abducting children for organ harvesting. He took Anjana, another missing girl, and was using the children for his criminal operations. However, he did not originally kidnap Ajay - Bhavana had already been holding Ajay captive for years before David became involved. David used the organ harvesting as a distraction while other kidnappings occurred.

Foreshadowing: David suggests a speech therapist for Ajay, which appears helpful but is actually part of his sinister plans.

Q6

What happened to Kathir in the underground room?

Kathir, Abi's husband, was killed by Bhavana when he discovered her during his search for Ajay. His decomposing body was found in the underground room where Bhavana had been holding Ajay captive. This occurred during the original kidnapping incident years earlier, and his death added to the mystery of what happened to Ajay.

Foreshadowing: Kathir is the one who initially finds Ajay outside the house during the first disappearance, hinting at his involvement in searching for the child.

  • kathir
  • death
  • underground room

Q7

Why did Bhavana offer Rhythm the poison choice between Ajay and her unborn baby?

Bhavana offered Rhythm an impossible choice - drink poison to save her unborn child or save Ajay - to make Rhythm experience the unbearable suffering Bhavana felt her entire life. It was her final twisted psychological attack, forcing Rhythm to choose which child to save, mirroring Bhavana's own childhood feeling of being ignored while Rhythm was favored.

Foreshadowing: Bhavana's confession reveals her deep resentment about childhood neglect, setting up this final act of psychological torture.

Q8

Did Rhythm survive the poison at the end?

Yes, Rhythm and her unborn baby both survived. After Bhavana shot herself, Rhythm managed to escape with Ajay and reach the hospital in time. The doctors confirmed that both Rhythm and her unborn child were safe, though weakened from the ordeal. Ajay also recovered and began speaking for the first time, calling out to his mother.

Foreshadowing: The film's tone suggests a bittersweet resolution rather than a complete tragedy.

  • poison

Q9

Why did Bhavana shoot herself at the end?

Bhavana shot herself rather than be captured or face the consequences of her actions. After her elaborate revenge plan failed and Rhythm refused to abandon Ajay, Bhavana was left with no escape. Rather than letting Rhythm win or revealing more, she chose to end her own life, taking her secrets with her.

Foreshadowing: Throughout the film, Bhavana is shown as someone who created a separate identity to cope with her pain, indicating she was capable of extreme actions.

  • bhavana
  • suicide

Q10

Why are insects such an important symbol in the movie?

Insects represent Rhythm's traumatic fear triggered by the kidnapping. The doctor explains that Rhythm developed a phobia of bugs after Ajay's disappearance. The insects appear during her flashbacks and panic attacks, symbolizing the creeping dread and psychological trauma that has consumed her life. The swarm on the ceiling at the clinic is what causes her collapse, linking her present suffering to the past trauma.

Foreshadowing: The opening scene with the sinister figure and the later discovery of the bouquet with intestines reinforce the insect symbolism as representations of death and decay.

  • insects
  • fear
  • trauma
  • phobia
  • symbol
  • flashback
Q11

What was the significance of the bouquet of human intestines?

The bouquet with human intestines and the 'Rest in Peace' card was David the doctor's handiwork, used as a deliberate distraction to confuse the police investigation. It suggested a ritual killing and drew attention away from the actual kidnappings. David admitted this was a calculated move to throw investigators off the scent while he and Bhavana operated separately.

Foreshadowing: The grotesque discovery intensifies the search for Ajay and creates the narrative of a serial killer, which is ultimately a red herring.

  • bouquet
  • intestines
  • distraction
  • david
  • card
  • confusion
Q12

How did Ajay survive all those years without speaking?

Ajay survived by being held captive in Bhavana's underground room for six years. The trauma of being kidnapped as a toddler and kept in isolation caused him to stop speaking entirely - a condition that required speech therapy. Despite the horror of his situation, Bhavana kept him alive, likely as part of her psychological manipulation of Rhythm. Ajay only regains his voice at the end when he calls out to Rhythm as they escape.

Foreshadowing: David the doctor recommends a speech therapist early in the film, hinting at Ajay's traumatic muteness.
