Aaron Boone begins to experience horrifying nightmares of a mysterious city named Midian. These visions unsettle him deeply and lead him to seek help from a psychotherapist.

Under the influence of his girlfriend Lori, Boone consults Dr. Phillip K. Decker. Decker manipulates him into believing that he has committed a series of gruesome murders, creating a sense of paranoia within Boone.

It is revealed that Dr. Decker is a serial killer who preys on families he views as 'breeders.' He devises a sinister plan to frame Boone for his own violent acts.

While wandering disoriented after taking drugs provided by Decker, Boone is struck by a truck and taken to the hospital. Here, he encounters Narcisse, who believes Boone is a critical figure in a prophecy related to Midian.

Narcisse, in a desperate act, tears his own face off to reveal his true monstrous form and informs Boone about the location of Midian. This shocking revelation sets Boone on a path to uncover his destiny.

Boone escapes the hospital and discovers Midian, a hidden sanctuary beneath a graveyard. Here, he meets Kinski and Peloquin, supernatural beings who debate Boone’s place within their community.

Boone's troubles escalate as he faces a group of police officers led by Decker outside Midian's gates. In a chaotic turn of events, he is shot by the police, falsely accused of being armed by Decker.

After being declared dead and revived in the morgue, Boone emerges transformed into a Nightbreed, thanks to Peloquin’s bite. This transformation marks a pivotal change in his identity and purpose.

Lori embarks on a mission to find Boone, entering Midian and encountering Sheryl Anne. She discovers the secretive and dangerous nature of the Nightbreed while becoming embroiled in their struggles.

Decker, now Lori's new boyfriend, tracks her down in Midian with deadly intent. Just as he is about to kill her, Boone emerges, impervious to death in his supernatural form, showcasing the extent of his transformation.

Lori learns from Rachel about the Nightbreed's past and their persecution at the hands of humanity. This knowledge deepens her understanding of Boone's new life and the stakes at hand.

Captain Eigerman leads a brutal militia assault on Midian, seeking to destroy the Nightbreed. Boone’s group is forced to defend their sanctuary against overwhelming odds.

Amidst the chaos, Boone commands the release of the Berserkers, a wild breed of Nightbreed. Their arrival turns the tide in the battle as they unleash havoc on their human aggressors.

In a climactic showdown, Boone confronts Decker. The battle culminates in Boone killing Decker, marking a significant retribution for his manipulation and the pain he caused.

As the remnants of Midian fall, Boone receives a charge from Baphomet to find a new sanctuary for the Nightbreed. With Lori by his side, they embark on a journey toward creating a new home for their kind.
