Max Matheson survives a devastating car crash that kills his fiancée Kayla, leaving him haunted by survivor's guilt and trauma. He undergoes psychological treatment with Dr. Beaumont, making slow progress as he tries to reclaim normal life.

Max's father Jack reopens the Mayflower Department Store in New Orleans and hires Max as a security guard. He's introduced to manager Keller Landreaux, buyer Jenna McCarty, and VP of operations Ryan Parker, as the store gears up for a new era.

Before his first shift, Max has a vision of a dead woman in a mirror. Jenna's reflection then rips off her head, and soon after, a shard of glass decapitates her in the shower. Max realizes he foresees deaths through mirrors.

On the following night, Max sees Ryan's reflection being eviscerated and tries to warn him, but Ryan dies in the same manner. Max rushes to Ryan's apartment to intervene but is too late.

After contacting his father to quit, Max sees Jack's face covered in cuts as danger closes in. He saves Jack from a reflective attack and chooses to stay at the Mayflower to keep his father safe.

Max returns to the Mayflower, where Detectives Huston and Piccirilli recount the Jenna and Ryan incidents. Max denies any involvement and is allowed to keep working, though the detectives remain suspicious.

At the main mirror, a flashlight guides Max to a box containing Eleanor Reigns's ID and a missing-person flyer. He contacts Eleanor's sister Elizabeth to learn more about her disappearance.

Max and Elizabeth uncover that surveillance files from Eleanor's disappearance were deleted the night she vanished, suggesting a deliberate cover-up by someone at the store. The evidence shifts suspicion to those who had access.

They visit Henry Schow, the last guard who accessed the files, whose reflection bears a scar. He reveals Eleanor's death and details how Keller raped and killed her, then coerced him into deleting the surveillance records to hide the crime.

Eleanor, now a trapped spirit in the mirror world, begins exacting revenge by manipulating reflections to kill Jenna and Ryan. Her vengeance escalates as Max learns he can see her and is forced to help.

Max and Elizabeth confront Keller and search the Mayflower basement for Eleanor's body. Keller chases them, nearly strangling Elizabeth, but Max intervenes, reviving the near-death connection and enabling Elizabeth to see Eleanor.

Max wrestles Keller into the main mirror, where Eleanor pulls him inside and kills him. The revelation shocks Max and Elizabeth as they witness Keller's vanishing into the mirror.

Back at the police station, Henry confesses his involvement in the cover-up. The police imply Keller is a fugitive for Jenna and Ryan's deaths. The interrogation room's mirror cracks, and Eleanor appears, killing Henry as the screen cuts to black.
