The film opens with the blessing of the new Little Magnolia School as guests look on. The mayor warns the school owner Stanley Pardo about a past incident in which a student died there. The atmosphere is tense, hinting that old ghosts may resurface.

Inside the school restroom, a ghostly boy appears to Faith, shrouding her in fear and foreshadowing the malevolent events to come. The encounter confirms that the past tragedy is not finished. Faith is unsettled and wary of the school’s aura.

The next day, Faith, Julio, and Stella mourn their daughters after the field-trip accident. Manolo Apacible offers life-sized talking dolls to help them cope with their grief. The offer is received with mixed feelings, setting the stage for a dangerous coping mechanism.

Julio quickly sees benefits from his doll and shares his progress with Faith and Stella. Faith initially refuses to accept a doll because of the resemblance to her daughter Maria, while Stella hesitates but is eventually persuaded to take a Maria-like doll.

Stella's husband Don is offered a doll by Manolo and, after his involvement, Stella reluctantly accepts one too. The families collectively begin to rely on the dolls to cope, though unease lingers about the objects' uncanny voices.

The dolls start to haunt those who adopted them. Disturbances involve Faith's housemaid, Stella's housemaid, Don, and a student of Julio, escalating the sense that the dolls operate beyond mere comfort. The terrifying visits deepen the characters' fear and grief.

The school principal reprimands Julio for bringing a doll to class, alarming both students and parents. Faith reveals she is pregnant, intensifying the emotional stakes. Stanley suggests disposing of Faith's doll to sever the influence.

Leonora, guided by the dolls, kills the school principal in a shocking act. The tragedy confirms that the dolls are more than comforting toys and become agents of murder. Fear and chaos spread through the school community.

Teresa launches a killing spree with Don and her mistress, targeting those around them. Don is left in critical condition and eventually dies from his injuries, intensifying the grief that binds the three families. Authorities arrive as Teresa threatens more victims and a loved one’s life hangs in the balance.

A housemaid's cousin, Augusto, is a witch doctor who conducts a séance to locate the haunting. He uncovers the name of Faith's ghost and learns Eldon Jacinto is Manolo's son, revealing that Eldon is the key to Manolo's control over the dolls. The revelation reframes the tragedy as a long-running vendetta.

Leonora lures Julio to Manolo's place and knocks him out as live dolls attack. The dolls kill Stanley, showing Manolo's reach and the real danger behind the seemingly innocent toys. The group realizes they must confront Manolo directly.

The two sides confront each other in a final showdown. Julio disrupts the ritual and battles the dolls; Manolo overpowers Augusto and impales Julio, but Julio returns to deliver a fatal blow to Manolo before dying. Faith and Stella give Eldon a proper burial, ending the immediate threat to their families.

In the aftermath, Faith gives birth to a child and Stella adopts Don’s illegitimate son, bringing new life to the families. The mid-credits scene shows another grieving parent being approached by Julio with a Leonora doll, suggesting the cycle may begin anew. The story ends on a note of unresolved vengeance and fragile hope.
