Lynn Blodgett, an artist with a deep-seated coulrophobia, is haunted by unsettling dreams that revive after her divorce from her husband, a doctor named Bert Tokyo, who physically assaulted her when he learned she sought a divorce and is now fighting for full custody of their son, Nicholas. The sleepless nights begin to bleed into real life when a naked-eyed, battle-axe–carrying clown appears outside her home in a chilling glimpse of a figure named Shivers The Clown. The morning after this ominous encounter, Lynn learns that a nearby family—where she had been house-sitting—has been massacred, an event that escalates her fear and questions about what is actually haunting her.

At a gallery showing of her work, much of it devoted to monstrous clowns, Lynn encounters Tucker Reid, a roller-coaster magnate who purchases one of her paintings for $8,000. Tucker Reid offers a personal tour and an invitation to his office, but a mugging interrupts the promising meeting; a man named Heston tries to assault them, and Lynn and Tucker manage to escape. The trauma from the attempted robbery lingers, and Lynn is shaken to the core when she glimpses the nightmare figure, Shivers The Clown, on her patio—a moment that prompts a call to the gallery owner and eventually a visit from the police, who dismiss her story as the product of fear and stress.

The gallery shifts into a financially charged turn when Lynn accepts an assignment to paint a portrait for twenty thousand dollars: the request is to depict a man’s father, a clown, and a convicted child molester. The money offers hope and security, yet it trails the same ominous thread that threads through the other events of her life. Meanwhile, in the background, Lynn’s husband, Bert Tokyo, presses for custody, child support, and a share of the profits from all her paintings since their marriage. His financial strain is evident—he has been out of work for months and has been exaggerating the seriousness of his job obligations.

Stalking intensifies as Shivers The Clown shadows Lynn while she is out with Tucker, and the clown violently murders Lynn’s friend Amanda. Detective Peters later informs Lynn of Amanda’s death and the growing belief that the clown terrorizing her is real, as grease paint is found under Amanda’s nails and smeared in bright patterns on a window. In a troubling turn, Bert agrees to meet with a man named Heston in a parking lot to discuss killing Lynn so Bert can collect life insurance. Heston pushes Bert for more money because the situation grows more complicated with Shivers’s presence and the police investigation.

A crucial reveal comes when Bert visits Shivers The Clown, who is disclosed to be Doug Richardson, a former patient of Bert’s and a sex offender with Leber’s congenital amaurosis. Bert tries to rein in the threat, ordering Shivers to leave Lynn alone, but the clown’s fixation with Lynn grows stronger. In a dramatic escalation, Shivers sends a party clown to Lynn’s house to distract a guard; the guard is beheaded, and Shivers spots Heston nearby. Heston then breaks into Lynn’s home and is shot with his own gun during a struggle with Lynn and Tucker, who had dropped by to check on her.

The tension peaks when Shivers murders Bert at the Tokyo residence after Bert’s attempt to bring Heston into the plot backfires, and the authorities drift toward the theory that Heston was the killer. Lynn remains unconvinced and, defiant, accepts a night out with Tucker to celebrate the substantial payment for her painting. But Shivers’s mission to “cure” his schizophrenia through Lynn continues unabated: he kills Lynn’s boss, abducts Nicholas after dismembering the babysitter, and breaks into a theater where he mutilates two employees.

As Shivers closes in on Lynn and Tucker, Detective Peters races toward the theater after receiving a 911 alert and learning that the DNA found under Amanda’s nails matches Doug Richardson. Shivers uses Nicholas to lure Lynn into the open, threatening to end her life. Tucker gains a crucial upper hand by blinding the clown from a projectionist booth, buying Lynn the precious seconds needed for the police to arrive and arrest Shivers.

Returning home, Lynn is left with a haunting postscript: a nightmare in which Shivers escapes from prison to attack her and Nicholas, a chilling reminder that the danger may not be over. The film keeps its focus on Lynn’s fragile psyche and the ever-present threat of a killer who hides behind a mask, forcing her to navigate a world where fear, custody battles, and a shocking medical history collide in a nightmarish struggle for survival.
