Shivers, whose real name is Doug Richardson, believes he can cure his schizophrenia by confronting Lynn with her deepest fear, clowns. Since Lynn suffers from severe coulrophobia stemming from a childhood trauma at a carnival, Shivers fixates on her as part of his delusional mission to 'heal' himself through forcing her to face her phobia.

Shivers The Clown is revealed to be Doug Richardson, a former patient of Lynn's ex-husband Dr. Bert Tokyo. Richardson is a sex offender who suffers from Leber's congenital amaurosis, a rare genetic eye condition. He escaped from an asylum and began his killing spree while obsessed with using Lynn's fear to cure his mental illness.

Bert Tokyo was Doug Richardson's doctor at the asylum before the events of the film. When Bert discovers Shivers is stalking Lynn, he visits the clown and orders him to stop, but this only fuels Shivers' obsession further since Bert represents authority who previously confined him. This connection becomes crucial when Bert tries to control the situation but ultimately fails.

Shivers kills Bert after Bert hires Heston to murder Lynn, which threatens Shivers' delusional plan to 'cure' himself through Lynn. Heston, hired by Bert to kill Lynn for the life insurance money, breaks into Lynn's home during the night. A struggle ensues between Heston, Lynn, and Tucker who had come to check on her.
