During the night Lynn experiences a vivid nightmare in which a young version of herself encounters a clown with a decomposing face after a carnival car crash. The vision underscores her coulrophobia and foreshadows the danger the clown will bring. The dream sets the tone for the otherworldly menace that threads through the story.

In the morning, Lynn learns that a family near where she was house-sitting has been massacred. The shocking incident heightens the sense that danger is watching her every move. It foreshadows the violence linked to the clown like figure haunting her.

At a gallery exhibit, Lynn meets Tucker Reid, a roller coaster magnate, who buys one of her monstrous clown paintings for $8,000. He invites her to visit his office after the tour. A mugger attack soon after forces them to flee for safety.

Lynn spots Shivers on her patio and faints. The gallery owner calls the police, who later question Lynn and dismiss her story as paranoia. The event deepens her sense that she cannot convince others about the danger.

Lynn reluctantly accepts a $20,000 commission to paint a man's father a clown who is a convicted child molester. The assignment intensifies the conflict with the gallery and with the implications of her art. It also ties her livelihood to a dangerous subject.

Lynn meets with Bert and his lawyer, who demand full custody of Nicholas, child support, and half the rights and profits to all the paintings since their marriage. It is revealed that Bert has been out of work and has been lying about his job responsibilities. His threats escalate the stakes of the legal fight.

Shivers stalks Lynn while she is walking with Tucker, and later murders Lynn's friend Amanda. Detective Peters contacts Lynn to discuss Amanda's death and notes evidence like grease paint under Amanda's nails and on a window, lending credence to her fears.

In a parking lot Bert meets with Heston and hires him to kill Lynn so he can collect life insurance. Heston pressures Bert for more money because of the complications caused by Shivers and the police. The plan sets the stage for the escalating danger.

Ignoring Bert's warnings, Shivers sends a party clown to Lynn's house to distract the guard. The decoy clown beheads the guard, and Shivers spots Heston nearby as the distraction unfolds. The threat grows more personal and direct.

Heston breaks into Lynn's home and a struggle ensues with Lynn and Tucker. In the end, Heston is shot with his own gun, ending that immediate threat. The confrontation reveals the tangled web of crime surrounding Lynn.

At the Tokyo residence, Shivers murders Bert after Bert tries to have Heston eliminate Lynn, threatening Shivers' imagined cure. The killing marks a major shift in the power dynamics and eliminates a key antagonist.

Shivers breaks into a theatre and uses Nicholas to lure Lynn out while he pursues them. Tuck blinds Shivers from a projectionist booth, and Peters and other officers arrive to arrest him. The theatre becomes the arena for the climactic confrontation.

Lynn goes out on a theatre date with Tuck to celebrate being paid $100,000 for the painting. The moment hints at a fragile normalcy amid the danger she faced. The couple clings to a preferred sense of life beyond the terror.

Back home, Lynn has a nightmare of Shivers escaping from prison and attacking her and Nicholas. The fear lingers, leaving the audience with a chilling final image of the threat that haunts her.
