Two muggers ambush The Man in a shadow-soaked London alley, searching for cash. He suddenly awakens with brutal, quiet intensity and defeats them in a savage, lethal display. The encounter establishes his predatory menace and sets the tone for the film.

Detective Neilson is pulled into the aftermath of a blood-soaked crime scene: the murders of Melissa and Nikki. As more victims appear, the detective senses a larger, darker pattern at work. The investigation threads these events together, hinting at a supernatural thread behind the violence.

In a stark flashback, Melissa and Nikki split off from the crowd at a goth club and Melissa brings The Man home. Melissa's grip on reality begins to fracture as she sees people morph into monstrous visages. A nightmarish creature bursts from Melissa's womb, ripping into Nikki and killing them both.

Sophie and Emma raid a house hoping to find loot. Emma uncovers a box of money, but an old man in the bed rises to attack. The two fight back and kill the man in a brutal struggle. Soon after, the dead reanimate: Emma and the old man return to stab Sophie, enacting a chilling cycle of vengeance from beyond the grave.

As victims are identified, Detective Neilson links the current deaths to Kemper, a former hypnotist and the son of a Satanist. Kemper's long history includes abducting and killing children for over a quarter-century, and he was moved between facilities for closer observation. The web of manipulation widens as the investigator digs deeper.

Nick Holland, living with an amputated left leg, visits an old friend for a transplant and steals Thomas's leg under medical supervision. After rehabilitation, he regains movement with the transplanted limb, believing he has claimed control over his body. The line between ambition and monstrous consequence begins to blur.

While driving with Natalie, Nick's prosthetic leg unexpectedly presses the accelerator, sending the car into a fatal crash that kills Natalie. Nick then stabs himself as police arrive, and The Man appears to slaughter the officers, tightening the cycle of violence. The horrific consequences ripple outward, linking Nick's actions to the broader terror.

Richard, a reporter obsessed with extreme pornography, discovers a live-streamed torture website and is fired from his job for abusing company systems. An anonymous sender leads him to a filming house, where he is murdered by two masked figures under The Man's orders. The grim discovery reveals that Richard is Detective Neilson's son, adding a personal stake to the hunt.

Fueled by rage, Detective Neilson storms Kemper's confinement and shoots Kemper in the leg and arm, but guards immobilize him. The Man, disguising himself as a guard, slaughters the facility director and the remaining guards. The brutal assault reveals The Man as Kemper's son and shifts the confrontation toward a personal reckoning.

In the brutal confrontation, The Man is exposed as Kemper's son, binding the monstrous thread between the killer and the family. The revelation intensifies the sense that violence is a repeating inheritance rather than a single act. The confrontation heightens the threat as the two face off.

Neilson shoots The Man, blowing away half of his head, and fires a fatal shot that ends Kemper's life. The dying reveal is chilling: The Man rises again, his remaining skull fissured and claws and tentacles slipping from the surviving portion of his head. The screen cuts away as he seems to claim Neilson in the end.

Although Kemper dies, the fragment of The Man suggests the cycle of horror may persist beyond the film's cutaway. The ending leaves Neilson's fate unresolved and hints that the dark pattern can return in another form. The film closes on a chilling note of ongoing violence.
