As a teenager, Dale watches his mother Eileen being murdered by his meth-addicted father. When the police arrive, they confront and kill the father. The officer who discovers Eileen runs from the house and vomits, and Dale re-enters to tell his mother that everything will be all right.

Years later in Las Vegas, Dale moves across the street from married couple Sarah and Josh. They introduce themselves by bringing over cookies. Josh explains that he is a croupier while Sarah is a graduate student studying the effects of pornography.

Sarah asks why Dale is staring; he says she reminds him of someone. That someone is later revealed to be his mother. This uneasy dynamic foreshadows their strange connection.

While Josh takes extra shifts at work, Sarah experiences a highly realistic dream in which she is murdered. When she wakes, her clothes seem different and the room is cleaned, prompting further investigation. She eventually discovers her original shirt bloodied in the laundry.

Josh suggests she talk to Leslie, a pregnant doctor. Leslie diagnoses her with night terrors and prescribes a hypnotic. Sarah is somewhat relieved, but she notices a prominent scar has disappeared, a detail Leslie cannot explain.

To escape the disturbances, Josh arranges a vacation to a luxurious hotel. There, Sarah sleeps well and does not experience any of the prior symptoms. The couple hopes the respite will end the night terrors.

Upon returning home, the night terrors and unexplained blackouts return, now accompanied by nausea. Dale notices that Sarah is ill and demands to know where she has been. The couple grapples with the returning disturbances.

Dale leaves for work; Sarah searches his house and uncovers photographs of people in deathlike states, including herself. She also finds a replica of the house key Dale had made during grocery deliveries. The discoveries deepen her sense of danger.

Dale returns home and apparently kills Sarah with a blow to the head. Sarah wakes in her own bed, gasping, and assumes it was another case of night terrors. The danger in their relationship becomes terrifyingly real.

At Leslie's house, Sarah notices an unused home pregnancy test showing that she is pregnant. Leslie promises to schedule more tests for the pregnancy, blackouts, and night terrors. The discovery deepens the stakes for Sarah's safety.

Unknown to Sarah, Josh is sterile. When he learns of her pregnancy, he leaves her. The couple's fractured relationship leaves Sarah increasingly isolated.

To understand the night terrors, Sarah installs hidden video cameras in her home. The footage reveals Dale entering the house, raping and murdering her, and then resurrecting her, during which she loses much of her memory. The disturbing loop confirms Dale's supernatural pattern.

Sarah researches Dale online and learns from Eileen that Dale has a long history of murdering and resurrecting neighbors. Eileen explains she caused herself to be institutionalized to escape Dale, who resurrected her after suicide. Eileen asks Sarah to kill Dale, an act she cannot bring herself to do.

Leslie gives birth and the baby does not look like either of the possible fathers. She recalls the video evidence of Sarah's rape and remembers Dale's grocery deliveries, hinting at Dale's broader reach. The odd birth deepens the mystery surrounding the family connections.

Josh returns after a friend convinces him to give Sarah another chance, but she shoots him, mistaking him for Dale. She seduces Dale to trick him into resurrecting Josh, and he revives him just long enough to rescue Sarah. Sarah then shoots Dale dead, and as he dies, all of his victims drop dead from their prior wounds, including Sarah, Josh, and Leslie, ending with a close-up of Leslie's child.
