A military convoy from Area 51 experiences a catastrophic accident on the highway outside Las Vegas, colliding with a car, which leads to the escape of a contained zombie. This creature wreaks havoc, infecting and killing numerous soldiers before advancing into Las Vegas, where it rapidly spreads the contagion throughout the city. Ultimately, military intervention fails disastrously, prompting the government to quarantine the entire area.

Fast forward six years, and we meet Bly Tanaka (Hiroyuki Sanada), a casino owner, alongside his aide Martin (Garret Dillahunt). The duo approaches Scott Ward (Dave Bautista), a mercenary and former Las Vegas resident, with a high-stakes proposition. They require his expertise to recover $200 million from his casino’s vault before the military detonates a tactical nuclear strike on the city. In exchange for his help, Tanaka promises Ward a whopping $50 million.

Desperate and grieving after having killed his own infected daughter, Scott reluctantly takes the job, assembling a team of his former comrades: Maria Cruz (Ana de la Reguera), Vanderohe (Omari Hardwick), the helicopter pilot Marianne Peters (Tig Notaro), the adept German safe-cracker Ludwig Dieter (Matthias Schweighöfer), and sharp-shooter Mikey Guzman (Raúl Castillo) along with his ally Chambers (Samantha Win). Together with Martin, they plan to infiltrate the casino.

Scott clandestinely decides to keep $15 million each for himself, Maria, and Vanderohe—each of whom shared a military past. This leaves Peters with a mere $2 million and Mikey with $500,000, while Ludwig’s share is only $250,000.

During this crucial mission, Kate (Ella Purnell), Scott’s estranged daughter working in a quarantine camp, directs them to Lilly (Nora Arnezeder), a cunning smuggler well-versed in navigating the city. Lilly further recruits Burt Cummings (Theo Rossi), a problematic camp security guard, by offering him $20,000. In exchange for her assistance, Scott entices Kate with the entirety of his $15 million share. However, knowing that Lilly had previously helped her infected friend Geeta (Huma Qureshi), Kate insists on joining the contingent to help find her.

Upon entering Las Vegas, they encounter a terrifying surprise: a zombie tiger. After a confrontation leaves Cummings injured, Lilly warns the group about a faction of intelligent zombies—dubbed the “Alphas”—who’ll grant safe passage in exchange for a sacrifice. The Queen, the Alpha female ([Athena Perample]), captures Cummings and takes him to the Olympus casino, where he’s infected by Zeus ([Richard Cetrone]), the leader of the Alphas. Lilly guides the crew to a building housing dormant zombies, which awaken upon exposure to light.

Through clever use of glow sticks, Ward path carves his way amidst the undead. However, chaos ensues when Chambers accuses Martin of treachery; her resulting predicament leads to her demise when Guzman has to detonate her hoarded weapons, obliterating both her and a swarm of zombies. Arriving at Bly’s casino, the team discovers that their mission has been compromised: Dieter uncovers blueprints indicating that previous teams had failed in this mission, unbeknownst to Martin.

As they proceed, the dynamics shift fatalistically when Ward’s daughter Kate chooses to search for Geeta, inciting a series of harrowing revelations. Confronted by a horde of Alphas, tragedy strikes when they kill Maria Cruz.

Martin’s betrayal becomes apparent as he traps the group, revealing his true motives. Just then, a tiger mauls him to death as he finds Lilly has turned against him. In the ensuing turmoil, Vanderohe faces off against Zeus but is overwhelmed. Amidst the chaos, Dieter sacrifices himself, ensuring Vanderohe’s safe passage into the secure vault.

As the countdown toward nuclear destruction accelerates, the remaining group reaches the rooftop. An intense confrontation with Zeus ensues, where Lilly distracts him with the Queen’s severed head, allowing Ward and Peters to make their escape. However, this act comes at a steep price—Lilly suffers a deadly blow from Zeus.

As the plot climaxes with the nuclear strike, Scott and Kate struggle against the unstoppable force of Zeus, suffering grievous consequences. The explosive finale sees the deaths of numerous team members and the heart-wrenching moment where Scott gifts his daughter money for her future before succumbing to his zombie transformation—prompting Kate to take his life.
