Rinko Tatsumi, a compassionate counselor at a crisis hotline, lives in a caring yet sexless marriage with her husband Shigehiko Tatsumi, a hygiene-obsessed workaholic who often keeps to himself. One day, an envelope lands at Rinko’s office, and opening it unsettles her: inside are candid photographs of her at home, some depicting her in intimate moments with sex toys. The shock deepens when she is contacted by the sender, Michiro Iguchi, a caller she once helped recover from suicidal impulses. Iguchi reveals that he has the photo negatives and will only return them if Rinko complies with his instructions.

Reluctantly, Rinko follows his orders. She begins by walking in public wearing revealing clothing, then buys and uses a vibrator, acting out the scenes Iguchi photographed in secret at home. In a way, Iguchi has sparked a sexual awakening in her, and he performs on cue, keeping his promise to return the negatives, but with one final demand: she must visit her doctor. At the clinic, a mark on her body clues Iguchi’s observation, signaling that she has breast cancer.

Rinko confides in her husband about the cancer and the planned mastectomy, but his reaction is one of shock and emotional distance, leaving her to face the medical news largely on her own. When Shigehiko finds one of the discarded photos, he begins to tail Rinko. He witnesses her outside in skimpy clothing, then in an alley, she masturbates in public; later, in the rain, she undresses completely and reaches orgasm, all while Iguchi photographs her from the window of a car. Shigehiko watches from the shadows and experiences his own conflicted arousal.

Iguchi escalates the events by luring Shigehiko to a warehouse, where he assaults him with a long mechanical appendage strapped to his waist, berating him for repressing Rinko’s desires and deriding the couple’s dynamics. Iguchi, himself already terminally ill, is furious that Rinko canceled her mastectomy because of Shigehiko’s resistance—and he intends to die knowing she remains entangled in her repressed urges. The confrontation ends abruptly as Iguchi dies from the progression of his cancer, leaving Rinko to confront the aftermath.
