Wrong Turn 4: Bloody Beginnings

The film opens at Glenville Sanatorium where the cannibals from previous films are kept as children. Using a hairpin stolen from Dr. McQuaid, they break free and unleash a brutal rampage that frees other patients and sets the hospital on edge. Their escape culminates in the murder of staff including Dr. Ryan and Dr. McQuaid.

Following the escape, the cannibals unleash a murderous spree inside the institution, killing an orderly, a guard, and the doctors who attempted to contain them. The hospital descends into chaos as the cannibals’ threat becomes legendary to anyone who might hear the stories. The timeline then shifts to present-day events.

In 2003, a group of nine college friends set out on a snowmobiling trip to a mountain cabin but become lost in a blizzard. Seeking shelter, they stumble upon the now-abandoned Glenville Sanatorium and decide to wait out the storm inside. Unbeknownst to them, the adult cannibals are hiding in the building, preying on trespassers.

Insomniac Vincent roams the asylum and comes across Porter's mutilated body. As the group processes the horror, Saw-Tooth strikes, killing Vincent by driving a metal rod through his chin and out the top of his head. The threat of the cannibals becomes personal for the survivors.

The group discovers Porter’s severed head wrapped in his jacket, amplifying their fear. Panic erupts as the cannibals strike from above, pulling Claire up with barb-wire from the balcony and decapitating her. The survivors realize escape may be impossible while the killers stalk them.

The cannibals strip the snowmobiles of their spark plug wires, ensuring the teens cannot escape by vehicle. The group is trapped by the storm and inside the sanatorium; Lauren volunteers to go for help while the others brace for further attacks. Tension mounts as the clock ticks toward morning.

Lauren sets out alone in the snowy landscape, hoping to find a way to summon help. Inside, the remaining friends reinforce their shelter and prepare to endure the night. The storm rages on as the cannibals continue to stalk the building.

The survivors barricade themselves in an office, studying files about the cannibals who lurk inside. Sara arms herself with a fire poker while Kenia, Bridget, and Jenna stand ready to defend the door. Daniel and Kyle search for more weapons, hoping to turn the tables on their attackers.

Daniel and Kyle find a hidden room filled with knives and saws and arm themselves for the confrontation. One Eye probes the office door with a drill, while Kenia, Bridget, and Jenna hold their ground. Three Finger lumbers nearby with an axe, heightening the sense of imminent danger.

The cannibals corner the group and force them into a cell. Kyle’s attempt to set them alight is checked by Kenia, who urges restraint. Three Finger uses a hairpin to pick the cell lock, and Saw Tooth delivers a single brutal punch to Kyle before the scene cuts to black.

Kenia returns to check on Kyle and finds him missing. The group later discovers a figure wearing a potato sack, only to realize it is Kyle with his tongue cut out, a grim misidentification that underscores the deadly stakes. The survivors mourn a teammate while planning their next move.

The remaining four girls retreat to the attic, don old winter clothes, and attempt to descend via a window. Most of the grate windows thwart their exit, and Jenna is killed with a drill as she makes a desperate escape. The group is driven deeper into the building’s maze as dawn approaches.

Morning reveals that Lauren has frozen to death just yards from a road. The cannibals chase Kenia, Sara, and Bridget on snowmobiles, closing in as the survivors scramble for safety. Bridget manages to stab one attacker, but the odds remain deadly as they fight to reach the outside world.

The pursuit culminates with Sara and Kenia crossing a barb-wire barrier, only to be decapitated by the barb that ensnares them. Three Finger then appears, grabs the girls’ heads, and tosses them into the back of a yellow tow truck. The cannibals climb in and drive off, leaving the fate of any survivors uncertain as the sun rises.
