The latest craze is $la$her$, a brutal Japanese blood sport game show where contestants can win millions of dollars if they survive being hunted through ever-changing arenas by costumed killers. All crimes are legal on the sets of the show, and the few rules that exist (such as staying motionless during commercials) are enforced by shock collars worn by both players and murderers. The current episode is presented as an all-American special featuring six contestants: Megan Lowry, Sarah Joslyn Crowder, Devon White, Tony Curtis Blondell, Michael Gibbons, Kieran Keller, Rick Fisher, Jerry Sprio, Rebecca Galley, Carolina Pla, and Brenda Thompson, Sofia de Medeiros. They are pursued by a new slasher, Preacherman, along with returning fan favorites Chainsaw Charlie, Neil Napier, and Doctor Ripper, Christopher Piggins.
A few minutes into the game, the six are scattered, with the lone cameraman, Hideo, Takaaki Honda, following Megan and Rebecca. The women are ambushed by Preacherman, but Rebecca retaliates, stabbing him in the eye with a makeshift wooden stake. The moment seems to promise a quick win for the survivors, yet the chaos escalates when Charlie and Ripper materialize, cutting Rebecca in half at the waist and menacing Megan, who had only joined the show to protest its ethics.
Megan returns to the group (with the exception of Devon) and they push onward toward the Bridge of Death, a perilous crossing above a pit of spikes. There, they meet Charlie again right as a commercial break begins. Rick reveals a fatalistic plan to save the others by throwing himself and Charlie off the bridge, which causes Charlie to panic and allow Rick to pass. When the broadcast resumes, Rick reneges on the deal, leaping off the edge and dragging Charlie down with him, leaving only Doctor Ripper standing.
In the next zone, Brenda is beheaded by Ripper, and Megan and Michael are chased into the Love Room, a zone where participants can be left alone if they consent to sex. Megan declines Michael’s advances, and he attempts to rape her, but Devon intervenes, his screams echoing from the room as he is drawn into the escalating violence. Devon discovers Michael’s body and is attacked by Ripper, whom he stabs in the ear with a stake. It’s revealed that Michael had faked his own death; he then kills Devon, seizes Ripper’s gear, and pursues Megan himself.
As Michael closes in, he reveals he is, in fact, a serial killer—the Bible Doll Killer—who joined the game hoping for a spectacular televised death, though he now senses a real chance to win. He corners Megan just as another commercial starts, bragging about his notorious history, confident that his on-air boasts are inadmissible in court. Megan counters by pointing out that the off-air footage captured by the constant cameras could still be used as evidence.
The break ends, and Michael lunges again at Megan, only to be shoved aside by Hideo. Grieved and enraged, Michael proceeds with his original plan to die on live television, decapitating himself as the audience watches. Hideo mocks him, noting that he could not tolerate “that sick maniac” being back on the streets with millions in winnings.
Megan, furious and determined, confronts the staff of $la$her$, vowing to spend her prize to bring the show down. She offers to pay Hideo to turn off the camera, a proposal that hangs in the air as the credits roll. The closing imagery reveals the sponsors behind the spectacle: cigarette brands Black Lung, Coffin Nails, and Cancer Man.
