Satya, an MBBS graduate, relocates to Kurnool with his mother and sister Nithya after securing a job as a government hospital doctor. He begins his new assignment and starts to settle into life in the city.

Guru's company manufactures poisonous saline that contaminates medical supplies. The hospital sees the deaths of three children linked to the saline, and the police refuse to act out of fear of Guru's power.

Satya lodges a formal complaint against Guru with the evidence he gathered from hospital cases. Guru learns of the complaint and attacks Satya at the Konda Reddy Fort, hanging him briefly. Satya is saved by his hospital dean, Dr. Robert.

Dr. Robert's intervention saves Satya, allowing him to leave the city with his mother and sister Nithya. The family departs to start a new life away from Kurnool.

Two years pass before Satya returns to Kurnool as the DSP, determined to wage a war against Guru and his men. He assumes a powerful policing role and begins dismantling Guru's network.

Satya learns that Dr. Robert died in an accident, a death he suspects Guru orchestrated. He vows to uncover the truth and bring Guru to justice.

Satya secures permission for a re-autopsy on Robert's body and digs up the grave. He discovers that the body inside is not Robert, and Guru reveals he killed Robert to help Satya escape; Rahim was killed for protecting the evidence.

The next day, Guru campaigns in the elections, but Satya arrests him after finding a flash drive that documents his illegal activities. The evidence strengthens Satya's case against Guru.

Swarna, Guru's wife, plots to kill Satya by stringing a garrote around his neck. Mahalakshmi, Satya's RJ girlfriend, is instead caught on the string and sustains a major neck cut, and Satya has her hospitalized; meanwhile Guru has been released from prison.

Encouraged by his mother, Satya calls Guru to the Konda Reddy Fort for a final showdown. The plan is to force a decisive confrontation that will end Guru's terror.

In a brutal hand-to-hand fight, Satya defeats Guru and hangs him in the same manner Guru had hung him before. The city breathes a sigh of relief as Guru's tyranny ends.

After Guru's death, Satya relocates to North India with Mahalakshmi, who survived her injuries, and his mother, continuing to work as both a cop and a doctor. He remains committed to justice and healing in his dual career.
